Hourly Pay Range:
$30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.Position Highlights:
- Position: Behavioral Health Clinical Case Manager I
- Location: LOH Naperville- Inpatient Adol
- Part time- 24 hrs/week
- Hours: Days
What you will do:
- Conduct foundational psychosocial assessments and gather collateral information to support development of treatment and discharge plans.
- Implement treatment and discharge plans in collaboration with the interdisciplinary team.
- Provide crisis intervention and supportive therapeutic interactions (individual, group, family) under guidance of senior staff.
- Coordinate discharge planning activities, including scheduling follow-up appointments and initiating standard referrals.
- Identify patient and family needs (emotional, social, financial) and escalate complex or high-risk barriers to senior case management staff.
- Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards
What you will need:
Required Education, Skills and/or Experience:
- Master’s Degree in Social work or a Master’s Degree in a recognized mental health field
- 1 year of experience in behavioral health environment preferred
If unlicensed, must be eligible to sit for licensing exam within the next 6 months as applicable. Must have obtained Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage Family Therapist (LMFT) licensure within one year of start date.
Preferred - Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT).
- Current CPR/BLS Certification issued by American Heart Association within 90 days of hire
- CPI (Crisis Prevention Intervention) certification within 90 days of hire
