Make an Impact in Inpatient Behavioral Health
Pyramid Healthcare at Duncansville is committed to providing comprehensive, client-centered treatment for adults facing Substance Use Disorders. As a trusted leader in addiction treatment across Pennsylvania, we take pride in our dedicated team and supportive environment focused on recovery and long-term success.
We are currently seeking a Case Coordinator to join our inpatient team. In this role, you will play a vital part in supporting patients’ transitions of care by coordinating aftercare services, maintaining communication with referral sources, and ensuring continuity throughout the treatment process.
Key Responsibilities
- Coordinate aftercare planning to ensure patients have access to appropriate next levels of care
- Schedule appointments, meetings, and follow-up services for patients
- Arrange halfway house interviews and placements as part of discharge planning
- Communicate with referral sources, families, and external agencies regarding patient progress (as appropriate)
- Obtain and maintain necessary documentation, releases, and records
- Support patients in navigating resources to promote successful recovery outcomes
- Maintain accurate records and documentation related to case coordination activities
- Collaborate with clinical, medical, and operational teams to support patient care
- Perform other duties as assigned
Qualifications
- High school diploma or equivalent required
- Experience in behavioral health, case management, or healthcare coordination preferred
- Strong organizational and time management skills
- Effective communication and interpersonal abilities
- Ability to work in a structured, fast-paced inpatient environment
- Basic computer and documentation proficiency
