Human Touch Home Health is looking for Occupational Therapists (OTs) for Full-Time, Part-Time, and PRN opportunities throughout Hyattsville and Northern Prince George’s County.
Occupational therapy at home is where independence becomes real.
You're helping someone get dressed in their own bedroom, safely use their own bathroom, prepare a meal in their own kitchen, and navigate the home they want to remain in.

What You’ll Do
As a Home Health Occupational Therapist, you will:

Complete comprehensive occupational therapy evaluations
-
Develop individualized plans of care based on patient needs and goals
-
Help patients improve independence with activities of daily living
-
Address bathing, dressing, grooming, toileting, meal preparation, and other functional activities
-
Provide therapeutic activities to improve strength, coordination, endurance, and functional ability
-
Assess home safety and identify barriers to independence
-
Recommend adaptive equipment and strategies
-
Provide transfer and mobility training related to functional activities
-
Develop individualized home programs
-
Educate patients, families, and caregivers
-
Complete reassessments and discharge planning
-
Collaborate with physicians and the interdisciplinary care team
-
Complete accurate and timely documentation using WellSky
What We’re Looking For
-
Active Maryland Occupational Therapist License Required
-
Graduate of an accredited Occupational Therapy program
-
Reliable transportation
-
Strong clinical assessment and communication skills
-
Ability to independently manage home health visits
-
Excellent organizational and time-management abilities
-
Home Health Experience Preferred, but Not Required
-
WellSky Experience is a Plus
