The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

Position Overview

ESSENTIAL FUNCTIONS

• Provides leadership, supervision, and operational oversight of licensed clinical staff and care management personnel, including Clinic RNs, Home Care RNs, Care Managers, Medical Assistants, Schedulers, Medication Room staff, and Medical Records/HIM personnel.
• Responsible for staff recruitment, onboarding, scheduling, retention, coaching, mentoring, clinical education, competency validation, performance management, and corrective action processes.
• Directs daily clinical operations and workflow to ensure efficient clinic flow, timely access to care, appropriate staffing levels, and effective utilization of clinical resources across the PACE center and contracted care settings.
• Oversees care management and clinical support services to ensure comprehensive participant assessments, care planning, care coordination, and ongoing monitoring of participant health status and service needs.
• Collaborates with Primary Care Providers, the Operations Director, Medical Director, and the IDT to ensure coordinated, participant-centered care and effective management of complex clinical situations.
• Maintains regular participation in IDT meetings, communicating participant status changes, contributing to care planning decisions, and facilitating the coordination of 24-hour care delivery.
• Ensures clinical staff practice within their scope of licensure, organizational policies, and established clinical standards while promoting accountability, professional growth, teamwork, and excellence in care delivery.
• Oversees clinical operations related to provider orders, medication management, documentation, medical records, treatment implementation, and communication of care plans and clinical instructions to appropriate personnel.
• Develops staffing plans and manages scheduling, workload distribution, on-call coverage, and deployment of resources based on participant census, acuity, and operational demands.
• Monitors clinic operations, appointment utilization, care coordination activities, and triage processes to optimize participant access, operational efficiency, and continuity of care.
• Serves as a clinical resource and decision-maker for staff, assisting with participant triage, acute care needs, advanced care planning, emergency situations, and complex clinical concerns.
• Collaborates with contracted providers, hospitals, skilled nursing facilities, assisted living facilities, home care agencies, laboratories, pharmacies, and community organizations to support seamless care coordination and participant transitions across care settings.
• Partners with the Quality and Compliance teams by supporting audits, implementing corrective actions, monitoring operational adherence to established standards, and assisting with regulatory surveys and readiness activities.
• Evaluates the effectiveness of clinical workflows, staffing models, and operational processes and implements improvements to enhance participant experience, staff efficiency, and care coordination.
• Provides direct clinical support and participant care when operationally necessary to ensure continuity of services, including assessments, triage, care coordination, and nursing interventions in the clinic, participant homes, and contracted facilities.
• Leads the clinical team during urgent, emergent, or high-acuity situations, adjusting staffing and workflows as necessary to ensure safe, timely, and effective care delivery.
• Perform other duties as assigned.

MINIMUM REQUIREMENTS

• Bachelor's degree in Nursing required. Master's degree preferred.
  Five years' clinical long-term care experience, including one year with frail elderly population, and three years of supervisory experience

• AHA BLS.

• Drivers License

• Registered Nurse – NJ or Registered Nurse – NLC multi-state
