Responsibilities:

  • Manage an assigned caseload and meet established productivity, billing, and service expectations based on client needs, program requirements, and regional service structure.
  • Complete comprehensive assessments and ongoing reassessments to identify client strengths, needs, risks, barriers, and goals across behavioral health, medical, social, housing, employment, education, financial, legal, and community-integration domains.
  • Develop, implement, and regularly review individualized treatment and care plans in collaboration with clients and relevant supports, including measurable goals, interventions, responsible parties, and expected outcomes.
  • Provide ongoing care coordination and case management services to monitor client progress, address barriers, coordinate services, and ensure interventions remain appropriate to identified needs.
  • Coordinate treatment, referrals and linkage to community resources and providers, including behavioral health, medical, psychiatric, housing, employment, education, transportation, benefits, legal, and recovery-support services, healthcare providers, behavioral health professionals, agencies, natural supports, and other community resources.
  • Collaborate with clients, families, treatment providers, schools, courts, probation/parole, social service agencies, and other community partners to support continuity of care and successful treatment and community outcomes.
  • Advocate with and on behalf of clients to assist with navigating systems, accessing resources, resolving barriers, and promoting client participation, self-determination, and recovery.
  • Monitor service delivery and client progress to ensure care plans remain appropriate, needs are addressed, and services are effectively coordinated.
  • Support continuity of care through treatment transitions, changes in level of care, discharge planning, and connection to ongoing community supports.
  • Coordinate transitions of care, including admission, level-of-care changes, discharge planning, continuing-care planning, referrals, and follow-up to support continuity of services.
  • Maintain timely, accurate, and compliant documentation in the electronic health record and complete required agency, Medicaid, grant, contractual, and regulatory documentation.
  • Monitor service delivery and client outcomes to ensure services are consistent with individualized care plans, treatment recommendations, program requirements, and applicable regulations.
  • Participate in interdisciplinary treatment teams, staffing, supervision, treatment-plan reviews, quality-improvement activities, and regulatory compliance activities as required.
  • Maintain competency in safety procedures, crisis response, emergency protocols, client rights, and applicable program and clinical requirements.
  • Provide authorized transportation to clients using a company or personal vehicle when required by the position and service needs.
  • Model and promote healthy coping, communication, problem-solving, self-advocacy, and appropriate interpersonal skills within interactions with clients, families, and community partners.
  • Maintain professional, ethical, and confidential conduct in accordance with HIPAA, 42 CFR Part 2, Montana law, and AWARE policies and procedures.
  • Provide safe, effective, and developmentally appropriate services using strength-based and evidence-informed practices, including responding appropriately to behavioral or safety emergencies.
  • Collaborate with clients, families, treatment teams, and community partners to communicate client needs, observations, progress, and recommendations and support coordinated care.
  • Complete required clinical, administrative, and electronic health record documentation accurately and within established timelines, including assessments, service notes, billing, referrals, reports, and other required records.
  • Maintain required training, certifications, and competencies and participate in supervision, staff meetings, team meetings, trainings, and other required organizational activities.
  • Manage assigned caseload, workload, travel, and productivity requirements to meet established program, service, billing, and client-care expectations.
  • Follow all applicable federal and state regulations and AWARE policies, procedures, and program requirements, and complete other duties consistent with the position as assigned.

Requirements

Talents, skills, and abilities:

  • Ability to assess and to respond to the needs clients. Requires a knowledge of case management and care management techniques, community resources, and knowledge of how to access services. Knowledge of a strength model perspective and an understanding of the family systems approach.
  • Ability to synthesize available resources and approach strategies into an effective plan that appropriately addresses the challenges presented by each customer. Ability to develop relationships with a diverse group of people; good interpersonal skills; sense of humor; adaptable; good oral and written communication skills.
  • Knowledge of substance use disorders, co-occurring disorders, recovery-oriented systems of care, ASAM criteria, levels of care, and community-based behavioral health resources.
  • Knowledge and understanding of care coordination principles, targeted case management requirements, client advocacy practices, and recovery-focused service delivery.
  • Knowledge of HIPAA, 42 CFR Part 2 confidentiality requirements, client rights regulations, and documentation standards.
  • Strong communication, organization, advocacy, documentation, assessment, problem-solving, and interdisciplinary collaboration skills.
  • Ability to maintain professional boundaries and manage confidential information appropriately.
  • Proficiency with electronic health records and documentation systems
  • Bachelor’s degree in a human services-related field preferred; or equivalent combination of education and experience.
  • Minimum of two years’ experience working with individuals with behavioral health, mental health, and/or substance use disorders preferred.
  • Must have, or be willing to obtain, a valid Montana Driver’s License. Must have an acceptable driving record; as well as an acceptable criminal and protective services background check.
