Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Primary Care Provider (Nurse Practitioner or Physician Assistant) for WelbeHealth PACE in Fresno, California.

POSITION OVERVIEW
• Provide comprehensive primary care for older adults enrolled in the PACE (Program of All-Inclusive Care for the Elderly) program
• Perform initial, semi-annual, and unscheduled participant assessments while managing acute and chronic medical conditions
• Develop individualized care plans in collaboration with physicians and the interdisciplinary care team (IDT)
• Participate in discharge planning and coordinate care following hospitalizations or long-term care stays
• Provide ongoing care for participants residing in nursing facilities through regular visits as clinically indicated
• Review utilization data, hospitalizations, and specialty referrals with the Medical Director to improve quality outcomes
• Participate in rotating evening and weekend call coverage

• Rotating Night & Weekend Call Required

Requirements

• Graduate of an Accredited Nurse Practitioner or Physician Assistant Program REQUIRED
• Active California Nurse Practitioner or Physician Assistant License REQUIRED
• Current NP Certification with Prescriptive Authority or PA Certification REQUIRED
• Minimum Two Years of NP or PA Experience REQUIRED
• Experience Working with Older Adult Populations REQUIRED
• Geriatric Experience Preferred
• Strong Knowledge of Chronic Disease Management and Geriatric Best Practices
• Ability to Participate in Rotating Night and Weekend Call
• COVID-19 Vaccination Required Prior to Employment
