Description
Duties may include:
- Assessing eligibility and the needs of individuals.
- Assessing possible individual risks.
- Interviewing individuals and their families.
- Facilitating team meetings.
- Coordinating service delivery among providers and developing and documenting person-centered plans using a strengths-based approach.
- Assuring quality service provisions.
- Maintaining compliance with all State of Virginia Licensure standards, Medicaid standards, and crisis intervention and prevention.
- Demonstrate effective oral and written communication skills.
- Have strong organizational and time management skills.
- Have proficiency in utilizing electronic health records and other software programs for documenting service delivery.
Preferred Qualifications:
- Master's degree in Human Services, Child Development, Early Childhood Special Education, Social Work, Counseling, Psychology, or related field.
- Three (3) years of related experience in assigned program area as a case manager, service coordinator, or multi-disciplinary team facilitator with children, youth, adults, and families.
