Job Description Summary:

The Ambulatory and Outpatient Registered Nurse (RN) Care Coordinator, a member of Advocate Health Nursing Professional Governance community, provides high- quality, patient-centered care through evidence-based practice, clinical expertise, and compassionate care delivery in an ambulatory care setting. The RN collaborates with interdisciplinary teams, advocates for patient needs, and upholds professional practice standards to promote optimal health outcomes.

We’re Looking For:

Completion of an accredited or approved Bachelor’s Degree program in nursing

Registered Nurse license issued by the state in which the teammate practices

Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association

Typically requires 3 years of experience in clinical nursing that includes experience in nursing assessment, care plan development and in educating patients

What You’ll Do:

Schedule: Shifts Scheduled Monday - Friday between 7:30am - 4:30pm to meet the department needs. Potential for occasional remote work. Schedule is based on the department need.

As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.

Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

• Supporting the practice - covering the in basket and working with patient calls.
• Coordinating pre and post procedure education and preparation
• Post procedure assessments for pain

The Interventional Pain Management Department specializes in the diagnosis and treatment of acute and chronic pain conditions using advanced, minimally invasive procedures. Our multidisciplinary team works collaboratively to improve patients' quality of life through evidence-based care, personalized treatment plans, and innovative pain management techniques. Services include spinal injections, nerve blocks, radiofrequency ablation, neuromodulation therapies, and comprehensive patient education, with a focus on restoring function, reducing pain, and supporting long-term wellness.

Knowledge, Skills & Abilities Required:

• Working knowledge of the components of quality and patient care needs specifically related to the area in which care will be performed.
• Maintains clinical expertise to effectively manage physical, emotional, psychosocial and spiritual needs of patients throughout the care continuum.
• Excellent analytical and interpersonal communication skills necessary to negotiate with families, patient, physicians and third party payers.
• Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.
