Health and Wellness Manager

Our Health and Wellness Manager is a multifaceted leadership role responsible for overseeing the daily clinical operations and well-being of residents in our memory care community. This person acts as a crucial link between the residents, their families, and the memory care staff, ensuring a high quality of life and compliance with all regulations.

Essential Duties

• Resident Care and Management: The Health and Wellness Manager is responsible for developing, implementing, and monitoring personalized service plans for residents. This includes conducting assessments, coordinating with healthcare professionals, and making sure that all residents receive the services they need.

• Creates a resident-centered approach to the planning and delivery of services to residents, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.

• Responsible for accurate completion and use of level of care determination tools to ensure residents receive the care and services they require and desire.

• Performs assessments on health, functional and psychosocial status of residents. Participates in the development of individualized service plans.

• Proactively manages care and services for each resident, maximizing the resident’s opportunity to remain in the assisted living environment.

• Problem-solves as part of the management team to case manage and meet the needs of residents. Assesses or evaluates residents with reported changes of condition. Provides team member direction, service planning, and coordination of care to meet the residents’ changing needs.

• Collaborates with the management team on the move in prospective residents, taking into consideration the needs of the prospective resident, current community acuity, and staffing levels.

• Provides in-person pre-move in assessments, if necessary. Ensures that resident care needs are met by coordinating services with other professionals.

• Partners in the investigation and root cause analysis of incidents. Collaborates with the management team on the development and implementation of appropriate interventions.

• Oversees the documentation and communication of resident care and services. Ensures resident records are completed in a timely manner and maintained according to company policies and state regulations.

• Staff Supervision and Training: Manages the health services team, which may include nurses, caregivers, and other support staff. This person is in charge of hiring, training, scheduling, and evaluating staff to ensure the highest standard of care and service. Provides on-call support and phone triage for team members’ questions regarding resident care. When needed, the RSD will provide floor coverage. The RSD will provide oversight to the staffing schedule.

• Community and Program Development: A key part of the job is fostering a positive and engaging environment. May design and implement programs that promote the physical, emotional, and social well-being of residents, such as fitness classes, social events, and recreational activities.

• Communication and Advocacy: Maintain open communication with residents and their families, addressing concerns, resolving conflicts, and providing updates on resident status. Provides education and support to residents and families. This person also acts as an advocate for residents, ensuring their needs and rights are protected.

• Regulatory Compliance and Administration: Ensure the memory care community adheres to all local, state, and federal regulations. Administrative duties may include managing budgets, overseeing medication programs, and maintaining detailed resident records.

Supervisory Requirements

The Resident Services Director is responsible for supervising and managing the entire health services staff either directly or indirectly.

Education and/or Experience

• Education/certifications in healthcare administration, nursing, or a related field.

• Managerial experience in a clinical setting.

• Experience in a senior living or long-term care setting.

• Exceptional organizational and problem-solving abilities.

• Strong interpersonal and communication skills to build rapport with residents, families, and staff.

• A compassionate and empathetic demeanor.

The employee must occasionally lift and/or move up to 75 pounds. Prolonged use of a desktop or laptop computer. While performing the duties of this job, the employee is regularly required to sit, stand, walk and talk, read or hear. While rare, travel by auto or airline may be required.
