At H2 Health, you'll work with a diverse outpatient population while delivering individualized, evidence-based care tailored to each patient's unique goals. You'll collaborate closely with physical therapists, occupational therapists, physicians, caregivers, and other healthcare professionals to help patients maximize independence and function. Your Role:
- Perform comprehensive speech-language evaluations and develop individualized plans of care.
- Assess and treat communication, language, cognitive-linguistic, voice, fluency, and swallowing disorders.
- Deliver evidence-based interventions designed to improve communication skills, cognition, safety, and overall quality of life.
- Collaborate with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal outcomes.
- Educate patients and families on treatment strategies, home programs, and techniques to support continued progress.
- Maintain accurate and timely documentation of patient progress and treatment outcomes.
- Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
- Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
- Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
