Program Coordinator - Memory Care - Full Time

GENERAL DESCRIPTION

The Memory Clinic/GUIDE (Guiding an Improved Dementia Experience) Program Coordinator reports to the Memory Clinic Manager and indirectly to the AVP of Outpatient Clinics. The coordinator provides support and services for the planning, implementation, oversight and continued growth of the GUIDE Program. This includes a wide range of tasks associated with the roll out and implementation of The GUIDE program. These tasks include coordinating and implementing plans that ensure the program runs within the required policies and procedures as outlined by the Centers for Medicare & Medicaid Services (CMS). The coordinator acts as the central point of contact managing internal and external communications. In addition to these duties the Program director continues to act as a Patient Navigator for the Guide program meeting with patients both in clinic and in their homes.

PERFORMANCE STANDARDS

1.        Supports the mission of the collaborative GUIDE program.

2.        Creates and distributes documentation in a timely and accurate manner.

3.        GUIDE database is consistently up to date and current.

4.        GUIDE policies and procedures are accurate and available at all times.

5.        Meets with patients and caregivers as required by GUIDE.

6.        Promotes adherence to GUIDE accreditation requirements and standards.

 

MINIMUM QUALIFICATIONS

1.        Bachelor's degree in public health or related health service field.

2.        1 year of related work experience.

3.        Experience with data collection and reporting.

4.        General knowledge of and skills in the use of computers, including basic computer applications.

5.        Certified in Basic Life Support within 3 months of hire.
