Human Touch Home Health is seeking compassionate and dependable Registered Nurses (RNs) for PRN home health opportunities throughout Fredericksburg and surrounding communities.

Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and nearby areas.

You do not have to cover the entire territory. Build your caseload around your availability and the areas that work best for you.

What You'll Do

• Complete skilled nursing visits in patients' homes
• Perform comprehensive nursing assessments
• Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
• Develop and update individualized plans of care
• Provide wound care, medication management, and disease education
• Monitor patients for changes in condition
• Educate patients and caregivers
• Communicate concerns and changes promptly to physicians and the clinical team
• Coordinate care with therapists and other disciplines
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Registered Nurse (RN) License Required
• Graduate of an accredited nursing program
• Current CPR/BLS Certification
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to work independently in patients' homes
• Home Health and OASIS Experience Preferred, but Not Required
• WellSky Experience is a Plus
