Cabrini Women's Mental Health is seeking a dedicated Intake Coordinator to be the first point of contact for individuals, families, and healthcare professionals, ensuring a seamless transition to our mental health services. Your empathetic approach and expert knowledge will guide clients towards the most appropriate care, while working closely with our multidisciplinary team.

Key responsibilities
Serve as the primary point of contact for individuals, families, and healthcare professionals seeking mental health services.
Conduct thorough assessments to determine the appropriate level of care and treatment options for clients.
Coordinate referrals and admissions, ensuring a smooth transition into our mental health programs.
Collaborate with the multidisciplinary team to develop individualised care plans and treatment goals.
Provide ongoing support and guidance to clients, their families, and healthcare professionals throughout their journey.
About you
You are a compassionate and dedicated individual with a strong commitment to quality patient care and outcomes. With your relevant nursing qualifications and registration as a Registered Nurse with AHPRA, you bring a wealth of experience in mental health and a passion for supporting women's mental health. Your excellent communication skills, leadership abilities, and positive attitude make you a valuable asset to our team.
You will have:
Relevant nursing qualifications and registration as a Registered Nurse with AHPRA.
Minimum 2 years' experience in a similar position, preferably in mental health.
Demonstrated leadership skills and the ability to work collaboratively in a multidisciplinary team.
Excellent communication skills, both written and verbal.
Strong organisational and time management abilities.
Flexibility and adaptability to changing situations and priorities.
