What You’ll Be Doing
You are the reason a resident who couldn't tolerate 30 minutes of PT last week is walking to the dining room this week.
As Fortis Care's Physiatry Advanced Practice Provider, you round at our partner skilled nursing facility in Savannah and see the residents who are stuck — the ones whose knee pain, shoulder impingement, spasticity, or neuropathic pain is keeping them from participating in therapy. You assess what's blocking them, you treat it, and you hand them back to the therapy team ready to work.
This is a 1099 independent contractor engagement. You set your own days, run your own panel, and are compensated on what you produce — not on hours logged.
Key Responsibilities
- Evaluate residents flagged by therapy or nursing as unable to progress — a focused functional, musculoskeletal, and neuromuscular exam to find the actual barrier.
- Treat it directly: peripheral joint and soft-tissue injections (knees, shoulders, hips), spasticity management including botulinum toxin, trigger point and dry needling, and targeted medication management.
- Build the plan with PT/OT, nursing, and the attending team — including DME, bracing, and adaptive equipment — so the functional gain sticks after you leave the room
- Follow through: recheck, adjust, document, and escalate or refer anything outside physiatry's lane.
- Teach: you become the facility's go-to resource on therapy readiness, mobility, fall prevention, and pain that's getting in the way of recovery.

Skills & Abilities
- Strong clinical knowledge of adult and geriatric medicine, with expertise in post-surgical, post-acute, and chronic disease management.
- Excellent communication and collaboration skills, with the poise to manage high expectations from discerning patients, families, and referring physicians.
- Ability to manage a patient panel efficiently while maintaining a high standard of care and an unhurried bedside manner.
Qualifications
- Current Nurse Practitioner or Physician Assistant license in the state of Georgia.
- For NPs: certification in Internal Medicine, Geriatrics, or Family Practice required. For PAs: current NCCPA certification required.
- Must have an unrestricted DEA license or wiling to obtain (we reimburse you)
- Minimum 1–2 years of NP or PA experience preferred; experience in subacute rehab, post-acute, SNF, or geriatrics strongly preferred.
- Ability to work independently in a facility setting with physician support.
- Familiarity with EHR systems and clinical documentation best practices.
Compensation & Engagement Terms
- Paid at per work RVU on the encounters and procedures you personally perform and document.
- Transparent monthly production reporting
- Paid on a bi-weekly cycle with a guarantee that we mutually set so no ramp to build your practic
- No cap. Your procedural volume — injections, spasticity management, dry needling — carries meaningful RVU weight, so a productive physiatry day pays substantially better than a standard rounding day.
