Schedule Details/Additional Information:

Monday - Friday 8am-5pm

  • Weekend on-call in person every 5th week rotation.
  • When providing weekend coverage, the APP receives a compensatory day off during the week.
  • There is no routine overnight call requirement.

In addition to inpatient responsibilities, there may be occasional opportunities to participate in outpatient PM&R follow-up clinics depending on departmental needs.

Pay Range:

$54.90 - $82.35

The Physical Medicine & Rehabilitation (PM&R) Nurse Practitioner serves as an integral member of our multidisciplinary rehabilitation team, working closely with physiatrists, nursing staff, therapists, case managers, patients, and families across the continuum of rehabilitation care.

The position is predominantly hospital-based, with approximately 90% of practice occurring in the inpatient setting. The APP rotates between two primary clinical responsibilities: the inpatient rehabilitation consult service and the acute inpatient rehabilitation unit. Rotations typically occur in blocks of several weeks at a time before transitioning to the alternate service.

Consult Service Responsibilities The APP evaluates new inpatient consultations from multiple hospital units, including neurology, trauma, orthopedics, neurosurgery, and general medicine services. Responsibilities include:

  • Performing comprehensive rehabilitation consultations.
  • Assessing functional deficits and rehabilitation needs.
  • Developing individualized rehabilitation treatment plans.
  • Making recommendations regarding therapy services, medical rehabilitation management, and rehabilitation candidacy.
  • Assisting with disposition planning and determining the most appropriate post-acute rehabilitation setting.
  • Collaborating with referring physicians, therapists, nursing staff, case managers, and families to facilitate optimal patient outcomes.

Acute Inpatient Rehabilitation Unit Responsibilities On the rehabilitation unit, the APP works closely with the attending physiatrists in the medical management of patients undergoing intensive inpatient rehabilitation. Responsibilities include:

  • Performing admission evaluations and facilitating admission processes.
  • Participating in daily patient management and interdisciplinary rehabilitation care.
  • Managing acute and chronic medical issues under collaborative physician supervision.
  • Reviewing, reconciling, and adjusting medications as appropriate.
  • Educating patients and families regarding diagnoses, treatment plans, rehabilitation goals, and discharge recommendations.
  • Coordinating discharge planning and ensuring appropriate follow-up care, referrals, and continuity of care after hospitalization.
  • Collaborating with therapists, nursing staff, case management, and consulting services to maximize patient functional recovery.

Patient Population The APP cares for medically complex rehabilitation patients. Approximately 75% of the patient population consists of individuals with neurologic diagnoses, including:

  • Stroke
  • Traumatic and non-traumatic brain injury
  • Spinal cord injury
  • Neuromuscular disorders

Additional patient populations include:

  • Multiple trauma patients
  • Amputees
  • Patients with significant medical complexity requiring rehabilitation
  • Orthopedic and musculoskeletal rehabilitation patients

Practice Environment This role requires strong collaboration with a team of physiatrists and advanced practice providers. The ideal candidate is a highly motivated, self-directed clinician with excellent clinical judgment, critical thinking skills, communication skills, and the ability to function effectively within an interdisciplinary team environment.

Previous experience in hospital-based practice is strongly preferred. Experience in Physical Medicine & Rehabilitation, neurology, and inpatient medicine would be particularly valuable.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED

License/Registration/Certification Required:

Active RN, and APRN/APRN-FPA/APNP or other APRN license(s) in state(s) of practice, AND

Active national board certification in area of clinical practice and populations served, AND

Active DEA registration prior to hire, AND

If Illinois practice: active Illinois Controlled Substance License prior to hire, AND

Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire

Education Required: ​Master’s Degree in Nursing

Experience Required: No experience required

Demonstrated high level of clinical proficiency, and excellent decision-making skills.

Demonstrated ability to work independently and as an effective member of a health care team.

Demonstrated ability to adapt to evolving technology and proficiency with the electronic medical record.

Excellent communication skills. Ability to effectively collaborate with and establish/build relationships with others.

Proven organizational skills and ability to prioritize effectively.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

Must be able to stand, walk, bend, stoop, and twist continuously throughout the workday.

Must have functional speech, vision, touch, and hearing.

Must be able to:

Lift up to 50 lbs from floor to waist.

Lift up to 20 lbs over the head.

Carry up to 40 lbs a reasonable distance.

Operate all equipment necessary to perform the job.
