Description
In consultation with the Executive Director, plans, develops and ensures delivery of meaningful recreational and social activity programs for residents of the LTC Home that reflect the organization's vision, strategic direction, resident centred approach and the residents' interests, ability and previous lifestyle. Ensures programs are in compliance with relevant government legislations and policies, with due consideration of the resources available.
Responsibilities:
Plans, develops and ensures delivery of meaningful therapeutic, recreational and social programs for residents of the LTC Home in accordance with the standards and requirements set out by the Ministry of Health and Long Term Care.
Supports staff in implementing innovative programs, including programs that are therapeutic in nature. Evaluates effectiveness of programs implemented
Coaches, develops, monitors and evaluates staff, promoting a strong, effective, engaged and efficient team. Encourages open communication and feedback within the team.
Assists in the development of internal budget and implements effective expenditure control system for the activation program
Ensures efficient and responsible use of resources
Coordinates with interdisciplinary team members to carry out and monitor resident care plan pertaining to activation, including charting and documentation. Actively and effectively contributes to the interdisciplinary team.
Ensures implementation of risk management, quality improvement programs and safe and hygienic environment for clients and staff
Participates and contributes to residents' team conferences
Coordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteers. Manages and evaluates volunteer performance and provides feedback as appropriate
In consultation with the Executive Director, develops, updates, communicates and implements policies pertaining to activation programs
Ensures up-to-date program records and reports, including accurate statistics and evaluation
Provides support to the Residents' Council including coordination of meetings and accurate minutes taking
Provides back up to the Social Worker as required
Coordinates the production of monthly Resident/Family newsletters
Coaches and prepares residents for special functions and programs arranged and initiated by the Yee Hong Community Wellness Foundation
Keeps up-to-date with developments and best practices in the field, and implements activation programs to reflect the values and principles of those projects
Other duties as assigned
Qualifications:
Bachelor degree in recreation and leisure studies, therapeutic recreation, kinesiology, or equivalent; B.S.W. and Registered Social Worker designation in Ontario an asset.
Minimum 3 years post qualification experience in program planning and delivery in health care setting. Knowledge of long-term care an asset
Knowledge and experience in counseling with seniors and their caregivers
Ability to supervise, coach and mentor a team
Excellent leadership qualities
Demonstrated initiative, problem solving and judgement skills
Creativity and innovation in developing programs and activities for the residents to maximize their independence
Superb customer service skills
Ability to contribute effectively and work as part of the interdisciplinary team
Must be agile and comfortable with change
Computer literacy. Knowledge of Chinese word processing an asset
Satisfactory Police Vulnerable Sector Screen result
Fluency in English and Chinese. Knowledge of Japanese, Filipino or a South Asian dialect an asset.
Physical Demands/Working Condition:
Occasional weekend or evening work
