Key Responsibilities
Patient Engagement
- Serve as a primary point of contact for patients, building trust and rapport
- Proactively reach out to patients to assess needs and ensure they feel supported
- Educate patients on available programs, services, and resources
Referrals & Care Coordination
- Identify patients who would benefit from additional services and facilitate appropriate referrals
- Coordinate with internal teams and external providers to ensure smooth transitions of care
- Track referral outcomes and follow up to confirm patients are connected and engaged
Documentation & Reporting
- Maintain accurate and up-to-date patient records and interaction logs
- Track engagement activity and referral data to support reporting and quality improvement
- Ensure all documentation meets relevant compliance and privacy standards
Qualifications
- Experience in a patient-facing, community health, or care coordination role
- Strong interpersonal and communication skills with a compassionate, patient-first approach
- Ability to manage multiple cases and priorities simultaneously
- Familiarity with healthcare systems, referral processes, or social determinants of health is a plus
