Primary Care APP - Merrimack

Job Summary:

St. Joseph Hospital, part of Covenant Health, is looking for an experienced Primary Care Nurse Practitioner to join our rapidly expanding Primary Care team in Merrimack, New Hampshire. The ideal candidate will have at least two years of experience in Family or Internal Medicine and the ability to manage a high-volume patient panel while achieving quality metrics.

Key Responsibilities:

• Collaborate with a diverse and successful team of primary care providers to deliver comprehensive care for patients of all ages.
• Provide preventive services such as routine checkups, immunizations, and management of chronic conditions.
• Create personalized care plans for patients and their families.
