Job Description

The Coordinator, Special Projects is responsible for surveillance, optimization, and outcomes associated with assigned special projects.  Collaborates across all departments to achieve project goals.  Creates and maintains partnerships with external organizations and local municipalities. Coordinates and supports utilization management, provider services, and referral processes of care to maintain and improve project outcomes.   Identifies and initiates quality improvement opportunities.  Maintains internal and external regulatory compliance. Supports projects in a manner consistent with overall hospital strategies. Engages with associated service lines and other organizational leaders and staff.  Provides analytical as well as strategic support in pro forma development.   Under general direction of leadership, plans and initiates project strategy with internal and external business partners.

6. Supports and/or leads projects related to partnership building and project optimization

7. Produces or collaborates with internal and external partnerships to create reports and identify associated requirements required to manage program and meet regulatory requirements

8. Analytical support for metric outcomes and project budgets

Qualifications

Bachelor's degree required; Master's degree in public health or healthcare administration is preferred

Minimum two years of project management experience including clinical and financial

Experience in pro forma development, strategy deployment, performance analysis, and intervention

Understanding of population health management concepts and strategies

Understanding of patient care workflows, specifically around care coordination

Experience in healthcare informatics

Experience in healthcare quality metrics

Demonstrated problem-solving and decision-making skills

Effective written and verbal communication skills.

Strong proficiency with Microsoft Office Suite (especially strong Excel skills) and EMR systems

Ability to work independently, complete projects on time, and initiate opportunities to improve processes Project Specific Qualifications:Public Hospital Redesign and Incentives in Medi-Cal (PRIME)

Experience with primary care setting

Experience with quality improvement, preferred

Knowledge of outpatient and inpatient healthcare, preferred

Salary Range:

$49.87 - $74.81 USD Hour
