We are currently seeking a Scheduling Coordinator to join our team serving the Ocala and The Villages market. If you are organized, detail-oriented, and enjoy helping coordinate care for patients and families, we'd love to hear from you.
What You'll Do
- Answer incoming phone calls and gather information needed to coordinate new patient referrals and admissions.
- Receive and process patient referral information from hospitals, physicians, facilities, and other referral sources.
- Obtain and verify essential patient information, including demographics, diagnoses, insurance information, physician orders, and Face-to-Face (F2F) documentation.
- Assist with coordinating the transition of patients from referral sources to home health services.
- Communicate with patients, families, physicians, referral sources, and internal team members to ensure a smooth transition to care.
- Work closely with clinical and administrative teams to ensure referrals are processed accurately and timely.
- Identify missing or incomplete information and follow up with appropriate individuals to obtain necessary documentation.
- Maintain accurate and timely documentation within the electronic medical record system.
- Help ensure new patient referrals are processed in accordance with agency policies and Medicare and regulatory requirements.
- Provide excellent customer service and represent VNA of Florida with professionalism and compassion.
- Perform other duties as assigned.
What We're Looking For
- At least one year of home health scheduling or coordination experience preferred.
- Medical office, healthcare administration, intake, or referral coordination experience is a plus.
- Strong organizational skills and exceptional attention to detail.
- Excellent verbal and written communication skills.
- Ability to manage multiple priorities in a fast-paced healthcare environment.
- Strong computer skills and the ability to learn electronic medical record systems.
- Ability to work independently while collaborating effectively with clinical and administrative teams.
- Professional, compassionate, and patient-focused approach to working with patients and families.
- Commitment to maintaining patient confidentiality and HIPAA compliance.
