Infection Preventionist (IP) – LVN

Tice Valley Post Acute is seeking a dedicated and experienced Full-Time Infection Preventionist (IP) – LVN to join our clinical team. This is a full-time salaried position responsible for leading and coordinating the facility's infection prevention and control program while supporting a safe, healthy environment for residents, employees, and visitors.

The ideal candidate is organized, detail-oriented, knowledgeable in infection prevention practices, and passionate about maintaining the highest standards of resident care and regulatory compliance.

Position Summary

The Infection Preventionist (IP) is responsible for planning, developing, implementing, evaluating, coordinating, and directing the facility's infection prevention and control program in accordance with current federal, state, local, CDC, OSHA, and applicable long-term care regulations and guidelines.

The IP works collaboratively with nursing leadership, facility departments, employees, residents, families, and government agencies to identify infection risks, implement appropriate interventions, monitor compliance, and promote a culture of safety throughout the facility.

Infection Prevention & Control

  • Plan, develop, organize, implement, evaluate, coordinate, and direct the facility's infection prevention and control program.

  • Ensure compliance with current CDC, OSHA, state, local, and applicable regulatory requirements related to infection prevention and standard/universal precautions.

  • Develop, maintain, and periodically update infection prevention policies, procedures, isolation precautions, and aseptic techniques.

  • Identify problem areas and develop corrective actions to improve infection prevention practices.

  • Interpret infection prevention policies and procedures for staff and departments as needed.

  • Maintain current infection prevention reference materials and resources.

  • Monitor infection prevention practices throughout the facility and ensure staff follow established procedures.

  • Conduct regular rounds of nursing units to identify potential infections, review environmental sanitation practices, and monitor isolation precautions.

  • Visit residents requiring isolation or infection control precautions to ensure appropriate practices are followed.

  • Review and analyze infectious disease laboratory reports and collaborate with the Director of Nursing Services regarding appropriate interventions and care planning.

  • Assist with the development and implementation of decontamination and sterilization procedures.

  • Monitor sterilization equipment, sterile packs, bundles, and supplies, including appropriate testing and shelf-life requirements.

  • Ensure appropriate handling, disposal, and removal of infectious and contaminated materials.

  • Maintain accurate records related to resident and employee infections.

Surveillance & Reporting

  • Prepare monthly summaries of resident and employee infections, corrective actions, and outcomes.

  • Prepare reports and summaries regarding suspected or confirmed healthcare-associated and community-acquired infections.

  • Provide the Director of Nursing Services and Administrator with written and verbal reports, findings, and recommendations regarding infection prevention.

  • Participate in investigations involving potential sources or outbreaks of infection.

  • Participate in surveillance activities and investigations involving residents and employees.

  • Report required communicable and reportable diseases to appropriate county and state health departments.

  • Monitor and assist with the facility's TB management and control program.

  • Maintain required infection prevention documentation, reports, studies, and records.

Department & Environmental Collaboration

  • Work with the Laundry and Environmental Services departments to establish and maintain appropriate infection prevention procedures.

  • Assist with procedures for handling clean and soiled laundry and linens.

  • Assist with routine and terminal cleaning procedures, particularly for isolation rooms and areas.

  • Assist departments in identifying job-related tasks involving potential exposure to blood or body fluids.

  • Ensure appropriate standard precautions and infection prevention practices are incorporated into department procedures.

  • Assist with discharge planning for residents requiring isolation or special infection prevention precautions.

  • Participate in facility surveys and inspections conducted by authorized regulatory agencies as directed.

Infection Control Committee

  • Serve on and participate in Infection Control Committee meetings.

  • Represent the Infection Control Committee as required.

  • Assist in evaluating and revising infection prevention policies, procedures, and aseptic techniques.

  • Provide written and verbal reports regarding infection trends and healthcare-associated infections.

  • Assist in evaluating proposed changes to cleaning products, equipment, and techniques.

  • Help implement recommendations made by the Infection Control Committee.

  • Provide information and recommendations regarding TB assessment, management, and isolation protocols.

Employee Health & Staff Education

  • Develop and maintain systems for identifying, evaluating, documenting, and reporting infections among facility personnel.

  • Assist with the employee health program, including employee counseling and education.

  • Provide employees with information regarding infection prevention policies and Hepatitis B vaccination.

  • Assist in establishing and maintaining TB Mantoux testing and employee recordkeeping protocols.

  • Orient new employees regarding infection prevention policies, personal hygiene, hand hygiene, standard precautions, and appropriate procedures involving blood and body fluid exposure.

  • Educate and reinforce staff compliance with infection prevention practices.

  • Monitor and report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals according to facility policies and procedures.

Administrative Responsibilities

  • Complete required medical forms, reports, evaluations, studies, charting, and other documentation.

  • Provide written and verbal reports and recommendations to the Administrator and Director of Nursing Services.

  • Maintain confidentiality of resident, employee, and facility information.

  • Participate in facility meetings, surveys, committees, and quality improvement activities as assigned.

  • Perform other duties as assigned by facility leadership.

Qualifications

  • Current, valid California LVN/LPN license in good standing.

  • Experience in infection prevention, long-term care, skilled nursing, nursing education, or clinical nursing preferred.

  • Strong knowledge of infection prevention principles, standard precautions, isolation practices, and regulatory requirements.

  • Excellent organizational, communication, documentation, and leadership skills.

  • Ability to educate and work effectively with nursing staff and interdisciplinary departments.

  • Strong attention to detail and ability to identify risks and implement effective corrective actions.

  • Ability to maintain confidentiality and exercise sound professional judgment.

  • Previous experience in a skilled nursing or long-term care setting is highly preferred.
