We are seeking a Home Health Aide (CNA) to join our compassionate Hospice team. In this role, you will provide essential care and support to patients and families during end-of-life transitions, ensuring comfort and dignity every step of the way.
What You Do
- Work 0.8 FTE. 64 hours bi-weekly
- Provide care during 8-hour day shifts
- Participate in on-call rotation every 4th weekend and 1 holiday per year
What You Need
- High School Diploma or equivalent
- Completion of a Home Health Aide/CNA program meeting state and federal requirements for placement on the WI & MN State Home Health Aide Registry upon hire
- Minimum of 1 year CNA experience in acute care, nursing home, or home health agency
- WI Registered Certified Nursing Assistant upon hire and MN Registered Certified Nursing Assistant within 90 days of hire
- BLS CPR certification from American Heart Association or American Red Cross with hands-on learning
- Valid driver’s license
