Partnership Lead

North Northamptonshire | Full Time | 37.5 Hours per Week | Permanent

Shape Stronger Partnerships. Create Better Outcomes.

At Change Grow Live, we know that the best outcomes happen when organisations work together. We're looking for a strategic and influential Partnership Lead who can build meaningful relationships across health, social care, criminal justice and community services, helping to create a more connected system that delivers lasting positive change for people and communities across North Northamptonshire.

Guided by our values to Be Open, Be Compassionate and Be Bold, we're looking for an experienced and influential Partnership Lead who is passionate about collaboration, system change and improving outcomes across health and social care.

Why This Role Matters

As Partnership Lead, you'll play a key role in ensuring people benefit from connected, responsive services that work together effectively. You'll build trusted relationships across Integrated Care Systems (ICSs), Combating Drugs Partnerships (CDPs), criminal justice services, health providers, local authorities and voluntary sector organisations, helping to influence strategy, drive collaboration and identify opportunities for innovation.

You'll combine relationship-building, project leadership and data-informed decision-making to help shape local priorities and strengthen integrated working across the system.

What You'll Be Doing

In this highly collaborative role, you will:

  • Develop a strong understanding of local Integrated Care System priorities, structures and developments.
  • Lead engagement with local Combating Drugs Partnerships and associated strategic workstreams.
  • Build meaningful partnerships across health, social care, criminal justice and voluntary sector networks.
  • Work alongside service leaders and data specialists to identify trends, opportunities and areas for improvement.
  • Project manage partnership initiatives, including externally funded programmes and development projects.
  • Support the effective coordination and reporting of OHID-funded activity and strategic outcomes.
  • Strengthen relationships with Primary Care Networks, Clinical Directors, Mental Health Transformation Teams, acute trusts, local place-based partnerships and VCSE alliances.
  • Act as a trusted advisor, supporting teams and stakeholders to navigate complex systems and work more effectively together.
  • Promote innovative, person-centred approaches that improve outcomes and reduce health inequalities.
  • Share learning, best practice and success stories to support continuous improvement across services.

Responsibilities

  • Map, monitor and understand the local Integrated Care System (ICS) and related activity.

  • Map, monitor and understand the local Combating Drug Partnership (CDP) strategy and outcomes framework.

  • Work with the service data lead to help coordinate data analysis to feed into ICS and CDP developments

  • Take on project management of any partnership developments including partnership projects funded through additional funding grants, and work with the services manager to appropriately allocate and developing funding areas.

  • Build and sustain healthy relationships with local partners and respond in a solution-focused way to challenges.

  • Develop relationships with CDP and sub-groups, Primary Care Networks including Clinical Directors, local place-based partnerships, Mental Health Transformation Teams, Acute trusts and VCSE alliance.

  •

Upskill staff and teams by providing information, advice, and guidance, that unpacks the complexities and enables services to integrate across criminal justice and the health and social care system.

  • Work as part of the service leadership team to monitor, capture and report on all OHID grant funded activity and provide any overview update to Service(s) Manager and commissioner reporting.

  • Apply a strength-based and system-focussed approach to working, sharing best practice and cascading any learning from positive service activity.
