At Riverside Home Health & Hospice, we have been serving patients and families in Southern Oregon for over 20 years. In recent years, we have expanded our Hospice services — providing comfort, dignity, and compassionate support during life’s most sacred moments.
Every hospice journey begins with one call.
We are seeking a Hospice Clinical Intake Coordinator who understands how we respond in that first moment truly matters.
This role plays a critical part in ensuring patients and families feel supported, informed, and cared for from the very beginning.

You work closely with hospitals, providers, and community partners
You coordinate admissions from referral to start of care
You ensure equipment and services are in place before arrival
You collaborate with clinical and operational teams

What You’ll Be Doing (High Level)
• Maintain patient medical records and documentation
• Verify benefits and obtain insurance authorizations
• Answer and route incoming phone calls
• Enter and update patient information accurately
• Receiving and reviewing hospice referrals
• Speaking with patients and families to gather information and explain services
• Coordinating with physicians and referral partners
• Confirming eligibility documentation and required orders
• Scheduling admissions and coordinating clinician assignments
• Supporting smooth transitions from hospital or facility to home
• Identifying and resolving barriers prior to admission
• Maintaining accurate and timely documentation
You serve as the bridge between referral and compassionate care.

• Healthcare, hospice, admissions, or intake experience preferred
