This role is ideal for a caregiver who excels at everyday support, providing comfort, and building trusting relationships with those they serve.

Schedule: Sat & Sun from 6pm – 8pm (Every or every other weekend)

Primary Responsibilities for PCW Milwaukee Jobs Under the supervision of an RN Case Manager, you will provide hands-on care and assistance with daily living activities based on a written Care Plan.

• The primary responsibility of this role is to assist the client with their evening routine and ensure a safe, comfortable transition to bed.
• Provide hands-on personal care, including assistance with bathing, personal hygiene, grooming, toileting, and dressing.
• Assist clients with basic mobility, ambulation, and safe, standard transfers.
• Plan and prepare nutritious meals and snacks according to the client’s dietary needs.
• Perform light housekeeping tasks to maintain a clean, safe, and healthy home environment.
• Report all observations and any changes in a client’s condition to the RN Case Manager.
• Accurately document all care and observations from your visit using our Electronic Visit Verification (EVV) system.

Requirements for Home Health Care Jobs Milwaukee

• Minimum 1 year of experience as a Personal Care Worker (PCW) or similar role is required.
• Previous experience in a home health care setting.
• A valid driver’s license and a reliable, insured vehicle to travel between client homes in Metro Milwaukee.
• Must have a positive attitude, be a team player, and be open to taking direction.
• Must be reliable, on-time, and able to keep a consistent schedule.
• Able to pass a pre-employment physical and drug screen.

Preferred Qualifications

• Experience caring for clients with conditions such as paraplegia.
• A solid foundation in basic PCW skills and a willingness to learn new care techniques.
