Summary of Position:
1.Preforms all the duties of a Pharmacist position without additional support or assistance.
* Documentation of completed Departmental orientation.
2.Prepares and dispenses all types Of medications using accepted standards of pharmacy practice for unit dose distribution, ambulatory pharmacy services and compounding of sterile and non-sterile products,
* No medication errors detected in routine Performance Improvement activities. Has shown competency in preparing sterile products by passing written and practical examinations.
3.Reviews medication orders for accuracy, completeness and therapeutic appropriateness.
* No medication errors detected in routine Performance improvement activities.
4.Addresses specific needs and characteristics of neonates, pediatrics, adolescents and geriatrics; can interpret data and carry out interventions appropriate to the patient's age-specific needs.
* Medications are prescribed appropriate to patient's age and weight.
5.Compares current medication therapy with new medication orders to prevent inappropriate drug use (drug allergies, drug interactions, duplicative drug therapy, sub-therapeutic or toxic doses.
* No medication errors detected in routine Performance Improvement activities. Preventable adverse dmg reactions avoided.
6.Monitors the effectiveness of drug therapy for individual patient cases
* Documents monitoring parameters and results.
7.Participates in multidisciplinary patient care rounds with other health care providers to discuss or explain drug-related issues.
* Documentation of 8 minimum of weekly participation in medical nursing, social service rounds or meetings.
8.Provides drug information to physicians and other health care providers, patients or their families.
* Documentation of services provided by intervention system or consult from nursing or medical staff.
9.Performs a detailed inspection of medication storage areas throughout designated unit/area in accordance with hospital and procedure.
* Conducted on a monthly basis.
10.Assists in the orientation and training of other pharmacy personnel in peer or subordinate relationships.
Participates in the training of pharmacy interns and residents.
* Participates in training process when assigned.
11.Supervises and directs the activities of pharmacy graduates and residents.
* Participates in training process when assigned.
12.Participation in performance improvement and other appropriate departmental committees. Identifies problems, finds and implements solutions. Involvement in one medication use evaluation per year.
* Documentation of participation.
13.Presents Inservice and/or orientation lectures to Pharmacy, Nursing or Medical staff at least once a year.
* Documentation of participation.
14.Performs admission drug history and drug discharge counseling for patients or family members as necessary.
* Chart or Pharmacy documentation.
15.Directs other pharmacy personnel, as assigned.
Education and Requirements:
Graduation from an accredited School of Pharmacy (BS or Doctor of Pharmacy).
PharmD or MS degree and/or completion of Pharmacy Residency Program may be used as a substitute for some experience.
Valid NYS Pharmacist license and registration.
Minimum of three years’ experience as a pharmacist in hospital setting or completion of Pharmacy residency.
Maintains professional competence through participation in continuing education programs and review of periodical literature.
Maintains and provides proof of 15 Continuing Education credits each year.
