- Document patient encounters in real time, accurately capturing histories, physical exams, assessments, and treatment plans under provider direction.
- Enter dictated information into the EHR (EPIC or assigned system) with a high level of accuracy and timeliness.
- Retrieve, organize, and prepare prior medical records, notes, reports, and test results to support clinical decision-making.
- Maintain strict adherence to the scribe scope of practice; does not act independently.
- Verify the accuracy and completeness of documentation, identifying and escalating discrepancies or missing information to the provider.
- Ensure compliance with HIPAA, patient privacy regulations, and site-specific documentation standards.
- Communicate clearly and professionally with physicians and clinical staff to anticipate workflow needs and support efficient patient visits.
- Demonstrate compassion, courtesy, and professionalism during patient interactions.
- Remain punctual, adaptable, and reliable in a fast-paced clinical environment, adjusting to schedule changes and clinic volume as needed.
Education
Required:
- High school diploma or equivalent
Experience & Skills
Required:
- Minimum of 1 year of experience in a medical or clinical setting or 1 year of undergraduate study in a medical field
- Prior experience with EPIC or other electronic health record systems preferred
- Typing speed of at least 55 words per minute with high accuracy
- Strong written and verbal communication skills
- Excellent attention to detail and organizational skills
- Customer service–oriented mindset with the ability to respond to patients and families with empathy and professionalism
- Ability to work collaboratively within a multidisciplinary healthcare team
- Commitment to patient safety, confidentiality, and ethical standards
