REFERRAL COORDINATOR - HOME HEALTH FULL TIME DAYS

Summary
Oversees the referral work queue in the system, faxes, and telephone referrals. Supports operations in all aspects of patient registration, insurance verification, and referral entry including quality assurance, accuracy of information, and completeness of documentation.

Job Duties

• Obtains necessary referral documents to ensure compliance with homecare and hospice regulations.
• Verifies patient information and documentation for accuracy.
• Registers patients needing services generated from physician and facility referrals.
• Handles insurance screening for patients and determines appropriateness for services.

Minimum Qualifications

• High School Diploma/GED
• Ability to articulate persuasively to both internal and external audiences.
• Flexibility to perform a variety of related and unrelated tasks throughout the day and work in a fast paced and frequently hectic environment.
• Knowledge of latest computer and communication devices to guide patients.
• Ability to establish and develop positive relationships with staff and leadership.

Preferred Qualifications

• Associate’s Degree Health care related field

Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
