LEAD COMMISSIONER – COMMUNITY REABLEMENT
Gloucestershire wants to appoint a lead commissioner for community reablement to work alongside operational managers to design, mobilise, pilot, and scale the Community Reablement Service over an 18-month period.
Role: The lead commissioner for community reablement leads commissioning, mobilisation, and scaling of community reablement in Gloucestershire.
Success: Success in the role is measured by successful mobilisation of an effective community reablement service delivering around support to around 4,000 people a year.
Working Arrangements: Flexible, working alongside operational teams as required. Some office based time each week is expected.
Duration: 18 months
KEY RESPONSIBILITIES
• Work collaboratively with operational managers and senior leaders.
• Translate strategic intent and the approved business case into operational service delivery.
• Develop and implement a Benefits Realisation Plan, covering financial and non-financial benefits.
• Establish clear performance, quality, and outcomes frameworks.
• Manage risks, dependencies, and key milestones.
• Provide regular progress reporting and recommendations to senior stakeholders.
SKILLS AND EXPERIENCE REQUIRED
• Senior commissioning experience within Adult Social Care.
• Strong understanding of reablement and domiciliary care models.
• Proven ability to translate business cases into operational delivery.
• Experience developing and implementing benefits realisation plans.
• Ability to work collaboratively across commissioning and operations.
