Lead Care Manager
Location: Santa Cruz, CA
Salary: $28.00–$35.00/hour
Overview
We seek an experienced Lead Care Manager to support Medi-Cal members through the Enhanced Care Management program. This field-based role requires direct community engagement with individuals facing homelessness, chronic health conditions, behavioral health challenges, and complex barriers to care. You'll work independently in members' homes and throughout the community, coordinating healthcare and social services to improve their health and stability.
Key Responsibilities
- Conduct member outreach, interviews, assessments, and eligibility reviews
- Develop and maintain individualized care plans based on member needs and goals
- Conduct regular face-to-face home and community visits
- Maintain ongoing contact via in-person visits, phone, and telehealth
- Coordinate services with healthcare providers, community organizations, and care partners
- Accompany members to healthcare appointments as needed
- Connect members with community resources (housing, food, transportation, financial assistance, healthcare)
- Track referrals and ensure successful service delivery
- Identify and address safety, behavioral health, and urgent concerns with crisis planning
- Complete accurate, timely documentation in the Electronic Health Record system
Minimum Requirements
- High school diploma or equivalent
- Medical Assistant diploma or Certified Nursing Assistant (CNA) certification
- Minimum 2 years of professional healthcare experience
- Valid driver's license with clean driving record
- Current automobile insurance
- Reliable personal transportation
- Proficiency with Microsoft Office, email, electronic documentation, and virtual communication platforms
- Ability to independently manage a field-based caseload
Preferred Qualifications
- Experience with Enhanced Care Management, CalAIM, Medi-Cal, or related programs
- Background working with vulnerable or underserved populations
- Knowledge of community resources and social determinants of health
- Bilingual English and Spanish fluency
Working Conditions
This position is 90% field-based, requiring extensive travel throughout the assigned county and frequent face-to-face interaction with members. You will work in member residences, healthcare settings, community organizations, and various community locations. The role involves frequent walking, standing, driving, and occasional lifting of supplies up to 20 pounds.
Compensation & Benefits
Salary: $28.00–$35.00/hour (based on experience and qualifications)
