Family Support Specialist – Help Families Build Stronger, More Stable Futures

Home Healthcare, Hospice and Community Services is looking for a Family Support Specialist to provide strengths-based, trauma-informed support to parents, caregivers, children, and kinship families throughout our community.


In this role, you’ll work directly with families through home visits, case management, resource navigation, parent education, advocacy, and community-based services. You’ll help families identify their strengths, address barriers, connect with essential resources, and build greater stability, resilience, and confidence.

In this role, you’ll have the opportunity to:

  • Build meaningful relationships with children, parents, caregivers, and kinship families.
  • Help families access healthcare, housing, childcare, transportation, education, financial assistance, and other community resources.
  • Support parents and caregivers as they work toward individualized family goals.
  • Provide education and guidance related to parenting, child development, resilience, and family well-being.
  • Collaborate with schools, healthcare providers, social service agencies, and community partners.
  • Support families during periods of stress, transition, crisis, grief, or other challenging circumstances.
  • Contribute to Family Resource Center programs, workshops, support groups, outreach, and community activities.


Key Responsibilities

  • Conduct home visits and provide family support services in accordance with program and agency standards.
  • Complete family assessments and help develop individualized, family-centered goals and service plans.
  • Maintain an assigned caseload and coordinate referrals, follow-up, and ongoing support.
  • Connect families with healthcare, behavioral health, early intervention, housing, employment, education, childcare, food, transportation, financial assistance, and other resources.
  • Provide support and resource navigation to grandparents, relatives, and other kinship caregivers.
  • Promote positive parenting, healthy child development, family stability, resilience, and self-sufficiency.
  • Respond appropriately to family safety concerns and serve as a mandated reporter in accordance with New Hampshire law and agency policy.
  • Maintain accurate, timely, and confidential electronic documentation.
  • Participate in team meetings, case reviews, supervision, training, and quality improvement activities.
  • Represent HCS professionally while building collaborative relationships throughout the community.

Requirements

Required

  • High school diploma or GED/high school equivalency diploma.
  • Successful parenting experience.
  • Experience negotiating community services and accessing resources as needed.
  • Knowledge of pregnancy, normal child growth and development, age-appropriate child nutrition, and play.
  • Ability to follow established family care or service plans and work within the boundaries of those plans.
  • Experience in human services, social work, psychology, counseling, early childhood education, or a related field.
  • Valid driver’s license and access to an automobile during working hours that meets State of New Hampshire registration requirements and applicable automobile liability insurance requirements.

Preferred

  • Associate or bachelor’s degree in human services, social work, psychology, or a related field.
  • Two or more years of experience working with children and families.
  • Experience in home visiting, family support, child welfare, or community-based human services.
