Description
Position Primary Purpose:
This position is responsible for leadership and oversight of non-provider primary care clinical services in addition to providing scheduled face-to-face office visits for case management, patient education, case management, triage and protocol driven treatment of select chronic and/or acute conditions. Administrative responsibilities will include oversight and evaluation of MA staff, oversight of vaccine, and 340b clinic pharmacy. Clinic work may include patient education, wound care, foot care/exams, and annual wellness assessments in addition to other duties appropriate for the position. This position description is created with the anticipation of significant expansion of primary care services and staff, which will require flexibility, creativity, delegation, and supervision.
Key Competencies:
Leadership and supervision (50%)
Ability to provide oversight, supervision, recruitment and evaluation of nursing and MA staff
Works together with clinic supervisor to set nursing and MA schedules
Ability to provide patient triage during routine clinic hours
Ability to provide oversight and management of vaccine and 340 b pharmacy inventory
Participation in clinic and institutional quality initiatives and programs
Clinical Services (50%)
Provides RN-delivered patient care within scope of practice, utilizing approved standing orders and clinical protocols for management of selected acute and chronic conditions with referral to clinical providers as appropriate
Work together and in support of providers in management of complicated patients including communication and working with specialty providers
Conducts medication review, assess and develop strategies to improve compliance
Provides patient education by provider referral
Provides wound care and dressing changes
Interest and ability to provide basic foot care
Provides and update care plans
Conducts comprehensive physical, mental, and psychosocial assessment per protocol
Adheres to standing orders and nursing protocols to order labs, referrals, medications, and provide education to support patient health improvement goals
Participation in mobile medical clinic
All other duties assigned
Population Based Health Responsibilities
Manage assigned patient population through proactive outreach, risk stratification, care planning, and coordination of care to improve health outcomes.
Minimum Requirements and Education:
Current Oregon RN license with a bachelor's degree or higher
Ability to work with a diverse population and a broad range of situations while maintaining a professional and courteous attitude
Complete all continuing education requirements
Minimum of three years' nursing experience or specialty certification
Experience in the delivery of primary care
Ability to work effectively with minimal supervision
Demonstrated collaborative leadership skills, including the ability to build trust, communicate effectively, support team-based decision-making, and partner with clinical and administrative staff to advance shared goals.
Strong computer skills including but not limited to the ability to send and receive email, write documents in MS Word, lookup information using the internet, and use the electronic EHR system
Maintain current BLS certification and complete annual blood borne pathogen training
Receive approval of the Medical Director showing complete knowledge of all the clinic's Acute and Chronic Care Nursing Protocols within six months of hire date
Complete White Bird new employee training
Able to function in a high-needs and at times chaotic environment
Preferred Skills:
Bilingual in English and Spanish
2+ years supervisor experience
Experience working with unhoused population and in a multicultural environment
Experience working at Federally Qualified Health Center
Minimum of two years' experience in primary care medical practice
Display knowledge of Chronic Disease processes including but not limited to diabetes, hypertension, asthma, COPD, hyperlipidemia
