We are strengthening our Santa Clara County team and are seeking a Physical Therapist who wants flexibility, independence, modern tools, and an organized home health structure behind them. You'll serve patients across South Bay's diverse neighborhoods.

Compensation & Benefits

• Salary: $140,000 – $190,000 per year (based on experience)
• Performance Bonus: Earn an additional 25% of your salary based on quality metrics

Your Role

• Independently perform physical therapy evaluations and reassessments
• Develop and manage individualized plans of care
• Deliver skilled therapy focused on mobility, function, fall prevention, and independence
• Collaborate with PTAs on an occasional, well-supported basis
• Communicate directly with physicians and the interdisciplinary team
• Educate patients and caregivers
• Complete documentation supported by AI tools
• Serve as the primary PT voice in patient care planning

Requirements

• Current California Physical Therapist license
• Valid California driver's license
• Strong clinical judgment, organization, and time-management skills
• Home health experience preferred; strong outpatient, inpatient, or rehab PTs are encouraged to apply
