Position Summary
We are seeking a dedicated and knowledgeable Wound Nurse (LPN) to oversee and coordinate wound care services for our residents. This position plays an essential role in promoting healing, preventing skin breakdown, and ensuring residents receive the highest quality treatment through evidence-based wound care practices.
Key Responsibilities
- Perform comprehensive skin and wound assessments on residents
- Monitor, evaluate, and document wound healing progress
- Develop and implement individualized wound treatment plans in collaboration with physicians and interdisciplinary team members
- Perform wound care treatments and dressing changes as ordered
- Identify residents at risk for skin breakdown and implement preventative interventions
- Maintain accurate and timely wound care documentation
- Ensure compliance with facility policies and clinical standards related to wound care
- Educate nursing staff on wound prevention, skin integrity, and treatment protocols
- Participate in resident care plan meetings and interdisciplinary team discussions
- Monitor and track wound care outcomes and quality measures
- Collaborate with physicians, nursing staff, therapy, and dietary teams to promote optimal healing
- Ensure wound care supplies are appropriately stocked and available
- Support infection prevention and control initiatives throughout the facility
Qualifications
Education & Experience
- Current, unrestricted LPN license in South Carolina
- Experience with wound care preferred
- Skilled nursing or long-term care experience preferred
- Knowledge of skin integrity management, pressure injury prevention, and wound treatment standards
Licenses & Certifications
- Current CPR certification
- Must maintain all required continuing education and licensure requirements
- Must remain in good standing with the South Carolina Board of Nursing
- Wound Care Certification is a plus but not required
Skills & Abilities
- Experience with PointClickCare (PCC) preferred
- Strong assessment and clinical documentation skills
- Excellent communication and teamwork abilities
- Strong organizational and time-management skills
- Ability to educate and mentor staff on wound care best practices
