As a Health & Wellness Coordinator, you will play a key role in helping residents thrive within our Retirement Villages by supporting their health, wellbeing and independence. You will work closely with residents, families and healthcare professionals to connect people with the services, support and community activities they need to age confidently and remain engaged in village life.

Your Opportunity
This is a part-time permanent position based at Cobbitty Village, working onsite preferably every Monday and Wednesday from 8:30am to 4:30pm.
You will support residents navigating My Aged Care, accessing community and health services, coordinating assessments and referrals, and maintaining strong relationships with allied health providers and external stakeholders. You will also coordinate resident wellbeing initiatives, education sessions and community activities that promote healthy ageing, social connection and independence.
Working closely with residents, families, Village Managers and Care Operations Managers, you will help residents access the right support at the right time, while maintaining accurate records, coordinating services, and contributing to future village initiatives
With empathy, professionalism and a resident-centred approach, you will:
Resident Wellbeing & Support
Build trusted relationships with residents, families and advocates
Support residents to access health, wellbeing and home care services that enable them to age in place
Conduct wellbeing assessments and develop individualized Health & Wellness Plans
Assist residents through the My Aged Care assessment and referral process
Coordinate referrals to appropriate care, community and support services
Advocate for residents and support them to navigate changing care needs
Events, Activities & Community Engagement
Coordinate and promote a calendar of resident education sessions, wellness programs and community events
Encourage participation in village activities that enhance wellbeing and social connection
Resident Experience & Customer Service
Deliver exceptional customer service to prospective, new and existing residents
Support residents through key transitions, including moving into the village and accessing care services
Respond to resident enquiries, emerging needs and concerns in a timely and professional manner
Administration & Documentation
Maintain accurate resident records and documentation within CRM systems
Coordinate scheduling, referrals and service records in line with organisational requirements
Prepare reports and support village compliance and regulatory requirements
Collaboration & Continuous Improvement
Work closely with village teams, pastoral care teams and external service providers
Contribute ideas and initiatives that improve resident outcomes and village services
Support a culture of continuous improvement and resident-focused innovation
What You'll Bring
You are a compassionate and organised professional who enjoys building relationships and helping older people live well.
You will also need:
Certificate IV in Social Work, Social Sciences, Health Management or a related discipline is good to have
Experience using care management systems, CRM platforms and Microsoft Office skills is a plus
Excellent written and verbal communication skills
Experience coordinating activities, events or community programs
Current Driver’s License may be required
