Description
- Provide comprehensive primary care services to geriatric patients in home-based settings
- Obtain detailed patient histories, perform physical examinations, and assess functional status
- Diagnose and manage acute and chronic medical conditions within scope of practice
- Order, interpret, and follow up on diagnostic tests
- Prescribe and manage medications in accordance with clinical guidelines and organizational standards
- Deliver patient and caregiver education focused on preventive care, chronic disease management, and safety in the home
- Provide medical care to a limited number of patients in nursing facilities to ensure continuity of care
- Support physician coverage in nursing facilities as needed
- Provide occasional outpatient clinic coverage in the Family Health Center based on operational needs
- Collaborate closely with CHMG physicians, nursing staff, Family Medicine Residents, and interdisciplinary team members
- Support and assist resident physicians in the care of geriatric patients
- Maintain accurate, timely, and thorough documentation in the electronic medical record
- Comply with all regulatory, licensing, and organizational policies and procedures
- Master’s degree in Advanced Practice Nursing or completion of an accredited Physician Assistant program
- Active/Eligible New Hampshire APRN or Physician Assistant license
- Current DEA registration
- National certification in specialty area preferred
- Minimum of 2–3 years of clinical experience in geriatrics or primary care for older adults
- Experience with home visits and/or nursing facility care strongly preferred
- New graduates will not be considered for this role
