What You'll Do:
Help develop, implement, and maintain social services policies and procedures.
Assess residents' psychosocial, emotional, and medically related social service needs.
Support residents and families with adjustment to illness, long-term care, discharge planning, and end-of-life concerns.
Maintain accurate social service documentation, progress notes, and required reports.
Provide guidance on Medicare, Medicaid, financial assistance, and available community resources.
Coordinate referrals and services with community agencies and healthcare providers.
Assist with discharge planning, including arranging equipment, home services, and agency referrals.
Participate in care planning meetings and interdisciplinary team collaboration.
Support continuous quality improvement by participating in departmental meetings and policy reviews.
Build and maintain positive relationships with residents, families, staff, and community partners.
What We're Looking For:
Education: Must possess, as a minimum, a high school diploma or its equivalent.
Experience: Prior experience in Social Services or similar role in a healthcare or post-acute setting preferred.
Compassionate Leader: Demonstrated commitment to resident-centered care and strong interpersonal skills.
Organized and Detail-Oriented: Excellent organizational skills and the ability to manage documentation and compliance requirements effectively.
Team Player: Ability to work effectively with multidisciplinary teams and establish positive relationships with residents and families.
