The Licensed Practical Nurse (LPN) provides direct patient care under the direction of a Registered Nurse (RN). This role supports patient assessments, care coordination, patient education, and communication with the healthcare team to ensure quality, compassionate care.Position Details:
Key Responsibilities
Assist with patient admissions, discharges, and ongoing care.
Perform patient assessments and monitor changes in condition.
Communicate patient updates and concerns to the RN and care team.
Participate in bedside handoffs and patient safety initiatives.
Provide patient and family education and support.
Document care accurately in the electronic medical record.
Collaborate with the interdisciplinary team to deliver high-quality care.
Delegate and direct support staff activities as appropriate.
Participate in unit meetings, huddles, and process improvement activities.
Perform additional duties as assigned.
Qualifications
Required
Graduate of an accredited Practical Nursing program.
Current LPN license in the state of practice.
Ability to meet all State Board of Nursing requirements.
Preferred
BLS certification (required within 30 days of hire if not already obtained).
ACLS, PALS, PEARS, S.T.A.B.L.E., or other unit-specific certifications as applicable.
LPN-IV certification (or ability to obtain within required timeframe, if applicable).
