Job Title: Physical Therapist (PT) – Full Time

Location: Brighton, MI
Setting: Skilled Nursing Facility (SNF)

Concept Rehab is looking for a motivated and compassionate Physical Therapist (PT) to join our team in Brighton, MI. This is a full-time opportunity for a PT who enjoys making a meaningful difference in the lives of patients while working alongside a supportive interdisciplinary team.

What You’ll Do

• Provide comprehensive physical therapy evaluations and treatments
• Develop individualized treatment plans based on patient needs and goals
• Help patients improve mobility, strength, balance, and functional independence
• Document patient progress accurately and efficiently
• Collaborate with nursing, therapy, and other healthcare professionals
• Educate patients, families, and caregivers on appropriate therapeutic techniques

What We’re Looking For

• Current Physical Therapist (PT) license in the state of Michigan
• Graduate of an accredited Physical Therapy program
• Strong clinical, communication, and organizational skills
• Compassionate and patient-centered approach
