Overview

Position Summary

The Food Is Medicine (FIM) Program Manager is a grant-funded leadership position supported through Delaware's Rural Health Transformation Program (RHTP). The Program Manager serves as the primary operational and implementation lead responsible for overseeing the development, coordination, execution, and continuous improvement of the Southern Delaware Food Is Medicine (SDFIM) infrastructure initiative, including workforce deployment, technology integration, culinary medicine capacity building, community-based interventions, care coordination systems, evaluation readiness, partnership management, and grant compliance.

Working closely with the Director of Community & Mobile Outreach, Food Is Medicine Registered Dietitian (RD), Community Health Workers (CHWs), healthcare providers, technology teams, community partners, academic institutions, contractors, and State of Delaware stakeholders, the Program Manager will lead implementation of the Food Is Medicine Infrastructure Initiative and establish the operational foundation necessary to support future Food Is Medicine pilot services and long-term sustainability throughout Sussex County. The position is responsible for ensuring successful achievement of program milestones, deliverables, reporting requirements, workforce development goals, and evaluation readiness activities outlined within the RHTP Food Is Medicine initiative. 

This is a grant-funded position. Employment is contingent upon the continued availability and renewal of grant funds.

Responsibilities

Program Leadership, Infrastructure Development & Operational Oversight

  • Lead all day-to-day implementation, coordination, and operational oversight activities associated with the SDFIM, ensuring compliance with approved grant deliverables, timelines, budgets, and reporting requirements.
  • Develop, maintain, and monitor comprehensive project workplans, implementation schedules, milestone tracking systems, risk mitigation plans, deliverable monitoring tools, and performance dashboards.
  • Coordinate implementation activities among Beebe Healthcare, TidalHealth, La Red Health Center, Sussex County Health Coalition (SCHC), Food Bank of DE (FBD), American College of Culinary Medicine (ACCM), Center for Nutrition and Health Impact (CNHI), Deloitte, and additional project partners.
  • Collaborate with the State-designated Component A contractor to align technology infrastructure, payment strategies, workforce development, reporting systems, evaluation frameworks, and sustainability planning activities.
  • Oversee implementation of Year 1 infrastructure-building activities and coordinate planning for Year 2 Food Is Medicine pilot implementation.
  • Lead sustainability planning efforts, including partner engagement, resource development, future infrastructure and funding strategies, philanthropic opportunities, and long-term operational planning.

Culinary Medicine Capacity Building & Education Coordination

  • Coordinate all American College of Culinary Medicine (ACCM) certification activities, including recruitment, enrollment, onboarding, participation monitoring, certification tracking, communications, and program completion.
  • Support implementation of Culinary Medicine Leadership Cohort activities and Community Health Educator certification pathways for Community Health Workers and other interdisciplinary staff.
  • Monitor progress toward workforce development goals, certification milestones, and training deliverables.
  • Coordinate training logistics, educational resources, participant engagement activities, and communication among Certified Culinary Medicine Specialists (CCMS), Registered Dietitians, CHWs, and healthcare providers.
  • Support development of future Train-the-Trainer programs and workforce expansion initiatives designed to increase Food Is Medicine capacity throughout Sussex County.
  • Facilitate development of a regional network of Food Is Medicine champions, culinary medicine practitioners, healthcare providers, and community partners.

Clinical Workflow, Epic Integration, & Care Coordination Infrastructure

  • Lead development, implementation, and continuous improvement of Food Is Medicine screening, referral, care coordination, patient tracking, and consistent service delivery workflows across participating organizations.
  • Coordinate Epic workflow design, patient identification processes, referral management systems, and operational activities collaborating with clinical, operational, and technology teams to ensure seamless integration across clinicians, Registered Dietitians, Community Health Workers, nutrition education services, and partner systems.
  • Develop and maintain standardized screening tools, referral protocols, operational guidance documents, workflow maps, training materials, and implementation resources.
  • Coordinate implementation of closed-loop referral systems connecting providers, Registered Dietitians, Community Health Workers, and community-based resources.
  • Support development of secure data-sharing processes, reporting structures, and partner communication systems that support care coordination and program evaluation.
  • Collaborate with evaluation and technology partners to establish dashboards, data collection tools, and performance monitoring systems.

Partner Engagement, Community Outreach & Resource Coordination

  • Serve as the primary operational liaison among healthcare organizations, community-based organizations, food access partners, academic institutions, contractors, consultants, and State of Delaware stakeholders.
  • Facilitate partner meetings, implementation workgroups, coalition activities, stakeholder communications, and follow-up on action items.
  • Coordinate development and maintenance of community resource directories, referral networks, food access resources, nutrition support services, and outreach partnerships.
  • Support CHW outreach efforts and community engagement strategies across clinical, community, faith-based, and underserved population settings.
  • Assist partners with implementation readiness activities, performance expectations, reporting requirements, and grant deliverables.
  • Foster collaborative relationships that support equitable outreach, rural healthcare access, and long-term Food Is Medicine sustainability.

FIM Service Operations & Nutrition Intervention Readiness

  • Coordinate readiness activities for Registered Dietitian-led nutrition services, medical nutrition therapy, nutrition counseling, culinary medicine education, and integrated Food Is Medicine programming.
  • Support development of participant eligibility criteria, enrollment pathways, referral procedures, clinical protocols, care coordination workflows, and implementation materials for future Food Is Medicine pilots.
  • Coordinate pilot planning efforts among healthcare, community, and academic partners to ensure successful implementation of future intervention models.
  • Facilitate and support testing and refinement of referral workflows, participant engagement processes, communication strategies, and operational procedures prior to pilot launch.
  • Monitor operational readiness milestones and ensure implementation activities remain aligned with grant requirements and approved timelines.
  • Collaborate with program leadership to identify opportunities for expansion and integration of additional Food Is Medicine services.

Data, Evaluation, Reporting & Performance Monitoring

  • Lead development and implementation of program performance measurement systems, dashboards, evaluation infrastructure, reporting processes, and continuous quality improvement activities
  • Coordinate evaluation governance activities, outcome measure development, data collection tools, reporting templates, survey instruments, and data-sharing agreements with partners and external evaluators.
  • Prepare and submit required monthly, quarterly, annual, ad hoc, budget, milestone, sustainability, and program performance reports in accordance with RHTP requirements.
  • Monitor progress toward grant objectives, workforce targets, infrastructure milestones, and performance measures.
  • Coordinate collection, validation, analysis, and presentation of programmatic, operational, and outcome data.
  • Develop executive summaries, reports, presentations, and stakeholder updates for internal leadership, funders, and external partners.
  • Facilitate continuous quality improvement efforts using data-driven decision making and performance monitoring processes.

Qualifications

Minimum Qualifications

�Bachelor's degree in public health, Nutrition/Food Science, Community Health, Healthcare Administration, Project Management, or a related field required.

�Five (5) or more years of progressively responsible experience managing nutrition programs, healthcare, public health, community-based, and/or grant-funded programs.

�Experience leading complex, multi-stakeholder initiatives involving healthcare organizations, community partners, academic institutions, and other key stakeholders.

�Demonstrated experience managing project plans, budgets, contracts, deliverables, performance measures, and program reporting requirements.

�Strong written and verbal communication skills, including experience facilitating meetings, presenting information, and preparing reports and other professional communications.

�Proficiency with Microsoft Office applications, including Word, Excel, PowerPoint, Outlook, Teams, and SharePoint.

Preferred Qualifications

�Master's degree in public health (MPH), Healthcare Administration (MHA), Nutrition, Population Health, Social Work, Community Health, Health Services Administration, or a related discipline.

�Experience implementing programs that address food insecurity, social drivers of health, behavioral change, and rural health populations.

�Experience utilizing Epic or other electronic health record (EHR) systems, healthcare data platforms, population health reporting tools, closed-loop referral technologies, or project management software platforms.

�Experience coordinating and supporting certification programs, educational and professional development activities, training, events, and community engagement initiatives.

Knowledge, Skills, and Abilities

�Strong program and project management skills, including planning, implementation, monitoring, evaluation, and continuous improvement.

�Knowledge of healthcare delivery systems, population health, care coordination, food insecurity, social drivers of health, and community-based services.
