As an Oncology Nurse Coordinator, you will play a key role in guiding patients and their families through every step of their healthcare journey. Using the nursing process, you will assess, develop, implement, and evaluate individualized care plans while proactively anticipating patient needs. Serving as the primary clinical contact, you will provide support, education, and guidance to patients, families, and caregivers, ensuring they feel confident and informed.In this role, you will coordinate multi-disciplinary and multi-site care, facilitate seamless communication among providers, and organize all necessary education and training for the care team. Utilizes the nursing process to assess, develop, implement, and evaluate individualized plans of care. Acts as the central point of contact for patient and caregiver inquiries related to care plans. Proactively anticipates patient needs to ensure timely navigation. Assesses learning needs and provides education to promote adherence to established care plans. Enhances the patient experience by delivering exceptional customer service and promptly addressing concerns. Performs all responsibilities aligned with the Nurse II role, with additional emphasis on care coordination, communication, and patient advocacy. Interacts with patients in person, via phone, virtual visits, and the patient portal to ensure care is well coordinated.

Starting hourly rate is $51.65. The actual compensation offered may vary based on a candidate’s qualifications, skills, and experience.

Qualifications:
Required Education and ExperienceLicense Requirement
License to practice as a Registered Nurse in the State of New York.
Certification in Basic Life Support (BLS) or eligibility for certification.
Education and Experience
1. Bachelor’s degree in Nursing and the equivalent of one (1) year of full-time experience in oncology nursing, medical-surgical nursing, or critical care; or
2. Bachelor’s degree in Nursing and the equivalent of three (3) years of full-time medical-surgical experience in an acute care setting; or
3. Associate’s degree in Nursing and the equivalent of three (3) years of full-time experience in oncology nursing, medical-surgical nursing, or critical care; or
4. Associate’s degree in Nursing and the equivalent of five (5) years of full-time medical-surgical experience in an acute care setting.
NOTES:
1. Incumbents must obtain and maintain applicable certification, as required by area of specialty and outlined in Departmental procedures, within thirty (30) months of appointment to this position and as a condition of continued employment.
2. As a condition of continued employment, incumbents must obtain certification in Basic Life Support (BLS) within six (6) months of their appointment to this title.
3. Incumbents must complete twelve (12) cancer-related contact hours per calendar year unless certified by the Oncology Nursing Certification Corporation (ONCC).
4. Degrees must be conferred by an accredited school, college, or university, or by an institution recognized by Roswell Park Comprehensive Cancer Center and/or Health Research Incorporated as adhering to acceptable educational standards.
