Job Summary:
The Housing Case Manager Lead supports the Housing Case Manager Supervisor by assisting with the day-to-day operations of housing programs, including administrative compliance, client navigation, housing coordination, building and maintaining community partner relations and workshop facilitation. A key part of this role is helping generate and organize reports, supporting the team when the Supervisor is unavailable and understanding the housing grants inside and out. This position plays a vital role in helping individuals and families experiencing homelessness achieve housing stability by supporting case managers, maintaining organized documentation, and ensuring strong relationships and communication across staff, landlords, and community partners.
Requirements
Job Responsibilities Include:
Case Management
- Maintain a caseload of Housing Grant-funded clients, providing all support services and prescribed activities to enable independence, such as Home Visits, case planning etc.
Client Support & Case Management Assistance
- Conduct/facilitate parenting classes with Housing Grant-funded families to help families grow their communications skills and parents increase their parenting skills
- As back-up to Case Managers, maintain communication and case support for clients when Case Managers are out of the office for extended periods
- As back-up to the responsible HCM, conduct Budgeting Class with Housing Grant-funded clients to help clients develop financial stability
- When needed, conduct Grant Intakes with prospective clients (all Housing Grants)
- Learn each current Housing Grant and support the team in meeting grant expectations
Data Entry & Compliance Management
- Perform internal organization and grant file and systems reviews to ensure compliance with FOS and Grant policies and procedures
- Support staff in maintaining HMIS compliance by reviewing data for accuracy, flagging missing entries, and ensuring timely updates to client records
- Execute monthly HMIS reports, understand where deficits might exist and work with each staff member to ensure appropriate data is updated in a timely manner
- Audit Case Management files regularly to ensure compliance and readiness for internal or outside review
- Help organize data for grant reports, outcome tracking, and compliance
- Create and maintain library of Housing Client Success Stories for use internally or externally
- Keep visual trackers or spreadsheets updated with client progress and goals
- Help Case Managers track and meet deadlines (assessments, housing goals, case plans, follow-ups) to ensure ongoing compliance with Grant and FOS policies
- Work collaboratively with team to ensure follow-through and solutions-oriented approach to any data deficits
- As needed, work with HCMS to develop and track departmental Key Performance Indicators (KPIs)
- Review and maintain SharePoint documents regularly to ensure team has access to most up-to-date documentation
Workshop, Training, & Program Support
- Work with the HCMS to plan and schedule workshops and training for both clients and staff
- Work with the team to develop and maintain weekly training schedule (sessions occur post-team meetings)
- Gather feedback after classes, enter responses into tracking tools, and share helpful trends or suggestions
- Research new community classes, training, or events that clients might benefit from and help connect them
- Work with the HCMS to build and maintain strong relationships with Community partners and Property Managers through regular communication
- Create documents that add value to drop-in and Housing Grant clients – such as a list of housing barriers by Property Management/housing development, etc.
Administrative & Management Support
- Serve as backup support for staff when the Housing Case Manager Supervisor (HCMS) is unavailable, ensuring continuity of client support and program coordination
- Represent the Housing Team and FOS by participating in FOS, Community, State and other management meetings as required
- Run weekly Department Team Meetings, including ensuring each team-member and each grant is represented; take Minutes and publish them within a timely manner and ensure action items are followed-up
- Undertake special projects as requested
Team & Community Collaboration
- Support a strong team environment by staying flexible and collaborative and by utilizing strong communications skills
- Help maintain communication between internal teams and the STG Housing Department
- Work with the HCMS to build and maintain strong relationships with Community partners and Property Managers through regular communication, personal visits, etc.
- When needed, field incoming calls and drop-in visitors; providing housing resources and support, connection to FOS departments or referrals to Community Partners

Pay : $20-24/hour DOE
