Day ShiftDescription:
Serves as a bridge between the health system and patients, particularly patients with Medicare and/or Medicaid and patients with identified social needs. Works to promote, maintain, and improve health and well-being through informal support and counseling to their patients. Uses culturally appropriate outreach, support, and navigation to help patients access health care and address identified social needs. Plays a key role in building individual capacity by increasing patient’s health knowledge and self-sufficiency through a range of activities such as health education, goal setting, motivational interviewing, and advocacy. Works collaboratively with other healthcare team and community-based providers to improve patient health outcomes and reduce racial, ethnic, and economic disparities in those outcomes.**This position requires a bilingual Spanish/English speaker (fluent read, write, speak)
As a Community Health Worker, you will be assessing, planning, implementing, monitoring, and evaluating actions to meet with clients health and human services needs, so that they can adopt health behaviors leading to self-sufficiency. You will be coordinating and providing client support that is safe, timely, effective, efficient, equitable, and client centered.
Requirements for working as a Community Health Worker
High School Diploma or equivalent.
Bachelor’s degree in a human services field, preferred.
Any combination of experience with health, education, and social services for a minimum of 1 year.
Basic understanding of health conditions, such as HIV/AIDS, Diabetes, COPD, Asthma, CHF, CVD
Bilingual - fluent Spanish speaking.
What Community Health Workers will do:
Communicating to clients the purposes of the program and showing them, the impact it may have on them.
Conducting intake interviews, enrolling and or referring individuals into community programs.
Assisting health care providers involved in the clients care, to ensure clients understanding of care plans.
Supporting the development, implantation, and execution of care plans.
Monthly reporting, maintaining electronic client files, including updated release of information, assessments, and other medical documents.
Travels extensively to client homes, community locations, agencies, and other outreach destinations.
