Job Title: Nephrology Social Worker / Dialysis Social Worker
Job Summary: We are seeking a compassionate and dedicated Dialysis Social Worker to assist patients and their families as they navigate the emotional, social, and practical challenges of chronic kidney disease (CKD) and end-stage renal disease (ESRD). In this role, you will perform psychosocial assessments, provide clinical counseling, assist with financial and insurance resources, and collaborate with an interdisciplinary renal care team to optimize patient outcomes and quality of life.
Key Responsibilities:
- Psychosocial Assessments & Counseling: Conduct comprehensive evaluations to identify patient emotional, cognitive, and social needs; provide supportive counseling to help patients and families adjust to dialysis treatments and lifestyle changes.
- Interdisciplinary Care Planning: Collaborate with nephrologists, renal dietitians, and dialysis nurses to develop and update individualized patient care plans.
- Resource & Financial Coordination: Assist patients with transportation arrangements, insurance coverage, Social Security Disability (SSDI), Medicare/Medicaid enrollment, and community support resources.
- Patient Advocacy & Education: Educate patients and caregivers on treatment options, advance directives, kidney transplantation processes, and coping strategies.
- Crisis Intervention: Provide immediate support and crisis management for behavioral, emotional, or logistical emergencies arising during treatment.
Qualifications & Requirements:
- Experience: Minimum of 2 years of professional Social Work experience [Required].
- Specialty Experience: Direct experience working in a dialysis center or nephrology setting [Required].
- Licensure: Active State Social Worker License (LMSW, LCSW, or equivalent) [Required].
- Certifications: Active Basic Life Support (BLS) certification through the American Heart Association (AHA) [Required].
- Core Competencies: Strong knowledge of ESRD regulations and resources, excellent crisis intervention skills, and exceptional empathy and communication abilities.
