Description
- Hours/Week: 16 Guaranteed Hours.
- Shift Schedule: Variable schedule covering Monday & Tuesday shifts (1 day covering Group 1: Mon/Wed/Fri schedule; 1 day covering Group 2: Tue/Thu/Sat schedule).
- Weekend / On-Call: None.
Position Overview:
Key Responsibilities:
- Renal Psychosocial Support: Conduct comprehensive psychosocial assessments and deliver specialized counseling for patients adapting to chronic kidney disease and end-stage renal disease (ESRD).
- Care Coordination & Advocacy: Facilitate discharge planning, connect patients to community renal support networks, and assist with practical/financial resource navigation.
- Multidisciplinary Collaboration: Partner closely with nephrologists, dialysis nurses, dietitians, and hospital care management teams.
- Clinical Documentation: Complete timely clinical assessments and progress notes adhering to acute care facility standards and state guidelines.
Qualifications & Requirements:
- Experience: Minimum 2 years of post-master's Social Work experience [Required].
- Specialized Expertise: Direct Dialysis / Nephrology Social Work experience [Required].
- Michigan State License: Active Michigan LMSW License active at the time of submission [Required].
- BLS Certification: Active American Heart Association (AHA) Basic Life Support [Required].
