Job Description Summary:

The Director, Care Coordination provides strategic, clinical, and operational leadership for MarinHealth's care coordination functions across the acute-care continuum. The Director is accountable for effective case management, utilization management, discharge planning, psychosocial coordination within assigned scope, and safe transitions of care. The role partners closely with physicians, Nursing, Social Work, Revenue Cycle, Health Information Management, ancillary departments, post-acute providers, payers, and community resources to support appropriate care progression, regulatory compliance, patient-centered outcomes, and responsible stewardship of resources.
The Director establishes department goals and performance expectations; develops and evaluates programs, policies, workflows, staffing models, and quality initiatives; and uses clinical, operational, financial, and experience data to identify opportunities and sustain improvement. The Director is also accountable for people leadership, budget management, regulatory readiness, and a culture consistent with MarinHealth C.A.R.E.S. standards.

Job Requirements, Prerequisites and Essential Functions:

Job Specifications:

Education

  • Bachelor of Science in Nursing (BSN) required.
  • Master’s degree in nursing, Health Administration, Business Administration, or a closely related healthcare field highly desired.

Experience

  • Minimum five (5) years of experience in an acute healthcare setting required.
  • Minimum five (5) years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required.
  • Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness.

Licenses & Certifications

  • California RN license (RN) required at hire.
  • Basic Life Support (BLS) required at hire.
  • Case Management certification (e.g., CCM or ACM) preferred.
     

Prerequisite Skills

  • Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment.
  • Knowledge of applicable healthcare regulatory, accreditation, payer, medical necessity, documentation, privacy, quality, and patient-rights requirements.
  • Demonstrated ability to lead managers and/or professional staff, manage change, coach performance, resolve complex problems, and build accountable teams.
  • Ability to analyze clinical, operational, quality, utilization, financial, and workforce data and translate findings into measurable improvement plans.
  • Demonstrated budget, staffing, productivity, and resource-management capability.
  • Strong written, verbal, interpersonal, facilitation, negotiation, and executive communication skills.
  • Proficiency with electronic health record, care management/utilization management, analytics, and standard productivity systems applicable to the role.

Primary Customer Served (Age Specific Criteria):

  • Primary populations served may include adolescents (12 to <18), adults (18 to <65), and older adults (65+), consistent with the services supported by Care Coordination.
  • Provides leadership for services supporting adolescent, adult, and older adult populations served by the assigned Care Coordination function.
  • Demonstrates and ensures staff competency appropriate to the physical, psychosocial, educational, cultural, developmental, safety, and transition-of-care needs of the populations served.

Patient Privacy (HIPAA Compliance):

  • This role requires access to protected health information (PHI) necessary to perform assigned Care Coordination leadership responsibilities. Access must be limited to the minimum necessary to perform assigned duties and managed in accordance with HIPAA, applicable privacy/security requirements, and MarinHealth policy.
  • Maintains confidentiality of patient, family, employee, payer, and organizational information and ensures staff follow applicable privacy, security, documentation, and information-sharing requirements.
  • Ensures appropriate use, disclosure, storage, and escalation of privacy or security concerns involving patient information within the department’s scope.

Duties And Responsibilities:
Essential Duties (Not Modifiable)

  • Provides strategic and operational leadership for care coordination, case management, utilization management, discharge planning, and assigned psychosocial care coordination functions.
  • Establishes, monitors, and advances department goals related to patient progression, length of stay, avoidable days, readmissions, denials, discharge timeliness, safe transitions, quality, patient experience, and other organizational priorities.
  • Develops, coordinates, and evaluates care coordination processes across the continuum from preadmission, when applicable, through hospitalization, discharge, and transition to post-acute or community services.
  • Ensures discharge planning processes identify patient and family needs, barriers to transition, post-hospital service requirements, education needs, and appropriate resources in a timely and patient-centered manner.
  • Oversees utilization management/review processes within assigned scope, including medical necessity, level-of-care review, payer requirements, documentation, escalation, denial prevention, and related performance improvement.
  • Partners with physicians, Nursing, Social Work, Revenue Cycle, Health Information Management, ancillary departments, and external partners to improve clinical and administrative coordination of care.
  • Uses data to identify trends, variation, barriers, and opportunities; establishes measurable action plans and evaluates whether interventions produce sustained improvement.
  • Leads quality assessment and performance improvement activities related to Care Coordination and participates in organizational quality, patient flow, utilization, and operational governance as assigned.
  • Ensures department policies, procedures, workflows, and practices remain aligned with applicable law, regulation, accreditation standards, payer requirements, professional standards, and MarinHealth policy.
  • Provides leadership for recruitment, selection, onboarding, competency, development, coaching, performance evaluation, corrective action, retention, and succession planning for assigned staff in partnership with Human Resources.
  • Establishes clear performance expectations and holds leaders and staff accountable for quality, service, productivity, documentation, compliance, teamwork, and C.A.R.E.S.-aligned behaviors.
  • Develops and monitors staffing plans, operational budgets, capital needs, productivity, and resource utilization; implements appropriate actions to support financial and operational stewardship.
  • Maintains effective relationships with post-acute providers, community agencies, payers, and other external partners necessary to support continuity and coordination of care.
  • Escalates significant patient safety, compliance, capacity, access, financial, or operational risks through appropriate leadership channels and ensures follow-through on corrective actions.
  • Performs other duties reasonably related to the responsibilities and operational needs of the position.

Compensation:

The posted pay range complies with applicable law and reflects what we reasonably expect to pay for this role. Individual pay is set by skills, experience, qualifications, and internal/market equity, consistent with MarinHealth’s compensation philosophy. Positions covered by collective bargaining agreements are governed by those agreements.
