We are seeking an experienced human services leader to join our Senior Leadership Team as Director of Care Coordination.
About the Position
The Director of Care Coordination provides strategic leadership and oversight for LEAP's established Case Management program while leading the development and implementation of Community Resource Coordination (CRC) services as Maine transitions toward the Lifespan Waiver.
This is a unique opportunity for an experienced case management or human services professional who enjoys both leading established services and building something new.
The Director will supervise and mentor LEAP's Case Management Assistant Director, provide overarching programmatic and regulatory oversight of Case Management, and serve as LEAP's organizational lead for CRC readiness, competitive procurement, program development, implementation, and quality.
Day-to-day supervision of Case Managers and routine Case Management operations will rest with the Case Management Assistant Director, allowing the Director to focus on senior-level strategy, leadership development, regulatory and quality oversight, program development, and emerging care coordination initiatives.
The Director also serves as LEAP's qualified MaineCare Section 13 Case Management Supervisor for services delivered to adults with developmental disabilities.
Requirements
What You'll Do
Lead CRC Development & Lifespan Readiness
- Serve as LEAP's organizational lead for Community Resource Coordination development under Maine's Lifespan Waiver.
- Monitor and interpret Maine DHHS/OADS, MaineCare, waiver, procurement, and implementation requirements.
- Lead organizational readiness planning and identify staffing, operational, training, quality, data, and implementation needs.
- Coordinate LEAP's preparation and response to competitive CRC procurement opportunities.
- If LEAP is selected as a CRC provider, lead development and implementation of the program from award through sustainable operations.
- Develop CRC workflows, policies, staffing structures, training expectations, documentation standards, quality systems, and implementation plans.
Provide Senior Leadership for Case Management
- Supervise, coach, and mentor the Case Management Assistant Director.
- Provide senior consultation regarding complex cases, regulatory interpretation, quality concerns, staffing challenges, and program improvement.
- Ensure LEAP's existing Case Management services remain stable, responsive, compliant, and person-centered.
- Maintain overarching programmatic and regulatory supervision required by MaineCare Section 13 while supporting the Assistant Director as the day-to-day operational leader of Case Management.
Drive Quality & Compliance
- Establish and monitor quality, compliance, and performance expectations across care coordination services.
- Support audit, licensing, certification, and contract readiness.
- Oversee documentation standards, HCBS compliance, MaineCare requirements, participant-centered practices, and continuous quality improvement.
- Analyze performance and quality trends and develop practical improvements when needed.
Develop People & Programs
- Partner with People & Culture and program leadership on workforce planning, recruitment, onboarding, training, and professional development.
- Help build a sustainable leadership structure for Case Management and emerging CRC services.
- Promote clear accountability, healthy delegation, professional growth, and sustainable workloads.
- Initially lead and supervise CRC staff as the program develops, with the ability to evolve the leadership structure as the service grows.
Build Strong Partnerships
- Represent LEAP in meetings and activities involving Maine DHHS/OADS, MaineCare, community providers, advocacy and resource organizations, and other stakeholders.
- Build productive relationships with individuals served, families, guardians, providers, and community partners.
- Participate in State meetings, provider forums, technical-assistance activities, and Lifespan implementation efforts.
Contribute to LEAP's Senior Leadership Team
- Participate actively in organizational planning and decision-making.
- Advise the Executive Director regarding care coordination strategy, regulatory developments, organizational capacity, quality, staffing, procurement, and implementation priorities.
- Collaborate across People & Culture, Finance, Operations, Training, Residential Supports, Shared Living, Community Supports, and other LEAP programs.
What We're Looking For
Required Education & Experience
- Bachelor's degree in social work, human services, public administration, healthcare administration, business, or a closely related field.
- At least five years of progressively responsible experience in case management, care coordination, IDD/Autism services, HCBS, MaineCare-funded services, or a closely related human services field.
- Meaningful experience in leadership, supervision, program development, quality, or compliance.
- Demonstrated ability to interpret regulatory requirements and translate them into effective program operations.
- Valid driver's license and ability to meet LEAP driving requirements when travel is necessary.
- Ability to successfully complete required background checks and LEAP hiring requirements.
Strongly Preferred
- Master's degree in a related field.
- Senior leadership experience in Maine IDD/Autism services or Case Management.
- Experience with program development or redesign, State contracts or procurement, program implementation, audit and quality systems, and staff development.
- Strong working knowledge of MaineCare, OADS, Sections 21 and 29, HCBS requirements, person-centered planning, and Maine's evolving Lifespan Waiver and CRC model.
Section 13 Supervisor Qualification
Because this position serves as LEAP's qualified supervisor for MaineCare Section 13 Case Management Services for adults with developmental disabilities, the successful candidate must meet applicable MaineCare supervisory qualification requirements.
In addition to having at least four years of experience working with intellectual disabilities or autism spectrum disorder, candidates must possess an applicable professional qualification such as LCSW, LCPC, LMFT, LMSW, APRN, Psychiatric Nurse, or Registered Nurse, or otherwise meet applicable MaineCare Section 13 qualification requirements.

Schedule & Work Environment
This is a full-time, salaried position based through LEAP's Central Office in Farmington, Maine.
The position generally works Monday through Friday during normal business hours and is designed to allow reasonable schedule and hybrid flexibility consistent with supervision, leadership meetings, State activities, procurement deadlines, operational needs, and LEAP policy.
Occasional work outside normal business hours may be necessary; however, routine weekend, overnight, and on-call responsibilities are not expected.
This is not intended to be a routine direct-care position or to carry a regularly assigned Case Management caseload.
Some travel to LEAP locations, community settings, State/provider meetings, and trainings is required.
