Oncology Social Worker

Job Description:

Utilizes social work expertise to address medical and clinical social work needs for a defined patient population. Performs psychosocial assessments and contributes to the interdisciplinary plan of care by developing and implementing a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. Works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties. May also be accountable for the management of discharge/transition planning needs in some settings.

Job Essentials

• Organizes and prioritizes daily work by screening and assessing new, current, and discharging patient needs in area(s) of responsibility.
• Completes documentation as required.
• Performs social work consultation for patients and caregivers.
• Conducts psychosocial assessments. Provides supportive, crises, trauma support, bereavement support, and other social work interventions as necessary.
• Educates, communicates, and coordinates with multiple patient care teams and outside agencies, as necessary, to ensure the patient receives quality care, without delays, and to identify and resolve delays and issues.
• Screens and conducts assessments when abuse, neglect, exploitation, or legal issues are suspected or identified. Consults with protective service agencies (i.e. DCFS; APS; Law enforcement) to ensure patient safety, appropriate and timely interventions, and a safe transition to the next level of care.
• Provides support and education to patients, families and staff members to facilitate in advance care planning (i.e. advance directives, referrals to palliative/supportive and/or hospice care).
  Serves as the mediator/broker between patients, families and the medical team, as needed.
• Consults with clinical, administrative, and support services to address and/or reduce quality, legal, and other concerns and promote patient and family-centered care.
• Completes timely and accurate documentation in the medical record, using knowledge of documentation standards for the department, to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing/reimbursement standards.
• May coordinate the discharge planning process from patient admission to departure. Screens patients and assesses transition planning needs. Contributes to the patient's interdisciplinary plan of care. Coordinates and refers to internal and external resources for needed services to facilitate a safe and timely transition to the next level of care.
• May supervise graduate students following guidelines and requirements established by their university and DOPL to provide an appropriate learning experience.
• May provide support and debriefing in conjunction with the Employee Assistance Program, for staff members following adverse events.
• May provide therapeutic modalities appropriate to the level of care.
• May maintain an active role in denial prevention. Proactively intervenes with payers to prevent inpatient denials. Communicates any necessary information to payers to help appeal existing denials.
• May ensure that productivity standards and expectations are met.
• May be credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.
  

Minimum Qualifications

• Master's Degree in Social Work from an accredited institution (degree will be verified).
• Current State Licensure as a Clinical Social Worker, (LCSW) or Certified Social Worker, (CSW)
• Computer skills (email, word processing, and spreadsheets).
  

Preferred Qualifications

• Experience working with in a healthcare setting.
   

• Hearing/Listening, Manual Dexterity, Seeing, Sitting, Speaking, Walking, Standing.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$35.60 - $54.93
