Oncology Social Worker
Full-Time – 1st Shift
BHCOR-Outpatient Oncology I Corbin, KY
The Oncology Pathway supports patients throughout the cancer care continuum by coordinating services across diagnosis, treatment, and follow-up care. The department works collaboratively with multidisciplinary clinical teams to ensure timely access to care, evidence-based treatment planning, and a patient-centered experience that promotes quality outcomes and continuity of care.
Under the direction of the Oncology Executive Director, the Oncology Social Worker is responsible for providing psychosocial services in collaboration with patients, caregivers, multidisciplinary team members and professional providers for Baptist Health cancer patients and their families.
Description:
Conduct comprehensive psychosocial assessments and develop patient- and family-centered care plans for oncology patients.
Provide evidence-based counseling, coping strategies, and emotional support while helping patients and families navigate the healthcare system.
Coordinate family meetings, advocate for patient needs, and connect patients with community, survivor, and support resources.
Develop and facilitate support groups while maintaining partnerships with community and national organizations.
Collaborate with the interdisciplinary oncology team to communicate care plans, support discharge planning, and improve patient outcomes.
Assist with educational initiatives for oncology staff, patients, and families.
Adhere to Baptist Health policies, infection prevention standards, safety protocols, emergency preparedness, and all required training.
Requirements:
Bachelor's degree in Social Work (BSW) required; Master's degree in Social Work (MSW) preferred.
Minimum of two (2) years of related social work experience required.
Current Kentucky Clinical Social Worker license required.
Must obtain Basic Life Support (BLS) certification through the American Heart Association within six (6) months of hire.
