Description
All Care Hospice is currently seeking a compassionate, organized, and dedicated Licensed Vocational Nurse (LVN) to join our team as a Hospice Care Coordinator in our Glendale office.
Position Requirements:
- Current California LVN License (required).
- Hospice Care Coordinator experience preferred.
- Strong communication, organization, and time management skills.
- Ability to coordinate patient care with physicians, nurses, and interdisciplinary team members.
- Comfortable working in a fast-paced office environment.
- Willing to make patient visits in the field when needed.
- Knowledge of hospice regulations and documentation is a plus.
Schedule:
- Full-Time.
- Monday through Friday.
